Mission

Families Creating Communities for Adults with Special Needs, Inc. is a not-for-profit organization founded to serve as a catalyst for creating sustainable housing, and supports solutions that enable adults with disabilities to fully participate in the community. It provides programs, training, Workshops and other educational opportunities for individuals with disabilities and their families for: long-term planning, readiness for independence, building essential life skills, and moving into housing settings in the community. It serves individuals and families primarily in the 5-county Philadelphia area.

Families Creating Communities for Adults with Special Needs, Inc. is a not-for-profit organization supporting individuals with disabilities ages 16+ and their families. Families CCAN empowers individuals of all abilities and their families to create self-directed, fulfilling lives in the community. Our work is guided by people with disabilities and the understanding that independence is different for each person. We provide learning opportunities, personalized support, and inclusive programs that create community, build social connections, and open doors to housing, employment, and civic life. We advance the strengths, goals and rights of all individuals to participate fully and meaningfully in their community.

DescriptionGrantsExpenses
GRANT-SUPPORTED INITIATIVESFamilies CCAN completed its 2-year grant with the PA Developmental Disabilities Council on a project to dismantle myths about housing for individuals with disabilities. The grant team included partner SAU1, a disability advocacy organization. The team wrote and delivered two virtual presentations and created an on-line resource publication. The presentation was given live to over 520 individuals, both virtually and in-person. The grant concluded on June 30, 2025. Grant funding from Lower Merion Township (Lower Merion, PA) is enabling Families CCAN to build an inclusive network of older adults and individuals with disabilities, called Lower Merion Village. This local connected web of neighbors and volunteers will bring resources, assistance, programming and volunteer opportunities to enable folks to successfully age in place. Building strong community ties provides a safety net for individuals with disabilities as well. The Village (not a true space but a metaphor) is connected to a national movement of Elder Villages. Project funding through this grant ended on June 30, 2025. Families CCAN continues to support this project.-$33,191
COMMUNITY PROGRAMS OFFERED FREE OF CHARGEWe sponsor free Monthly Resource meetings on a virtual platform. We hosted a total of 11 meetings in FY2025 on topics such as: Financial Planning Basics, Busting Housing Myths, Health and Wellness, Transition from High School. Average attendance is 35 families, individuals and service providers/professionals. We often feature individuals with disabilities teaching from lived experience.We offer a popular free cooking program called Families CCAN Cook. We have now hosted over 140 sessions of this virtual instructor-led cooking session designed to help individuals with disabilities learn cooking skills and make dinner in 1 hour. Participants join from their own kitchens and learn how to measure, cut, chop, use utensils and kitchen tools, and some nutrition basics. This program draws participants from across the state of Pennsylvania. Weekly attendance has grown to an average of 14 participants per session.For the 3rd year we have hosted quarterly, in-person Meet-ups for individuals and families living in and adjacent to Delaware County, Pennsylvania. These free gatherings promote connections, relationship-building and resource-sharing in a more localized way.-$30,559
WORKSHOPSFamilies CCAN supports families and individuals to address long-range planning and readiness using a person-centered and strengths-based approach. Our Peer to Peer Workshop employs peer instructors to teach other individuals with disabilities how to successfully live in the community, with supports. Instructors are currently living in the community and provide important guidance on: How to manage and budget funds, how to organize time and space, how to plan and prepare meals, and other topics that surface. We offer this virtual 5-session Workshop 3x per year and have 6-10 enrollees (individuals with disabilities) each time. We sponsored a virtual Empowering Independence Workshop for individuals with disabilities along with a family member. This 5-session Workshop is offered for a fee and is very popular with transition-aged youth planning for post high-school. We focus on creating a vision for independence based on an individuals strengths and interests, identifying important people for supports, identifying readiness goals & practicing steps to meet them, and making the planning ahead process more manageable.-$27,444
ADDITIONAL SERVICES OFFERED Families CCAN provides individualized consultation services to support families and individuals through Family Navigation. Trained staff provide guidance for issues such as: establishing direction for independence, finding resources, building community connections or creating a Personal Support Network.
