Mission

The purpose of the Corporation is to solicit, collect, receive, accumulate, administer, and disburse funds to further educational, community, and social development, and health purposes.

Potts Know and Grow- Self-Healing Communities is an evidence-based model for identifying and addressing a community's needs and strengthening its core family structure. Pathways began learning about this model and needed work in 2019, and it has continued to gain momentum in rural areas of the state. One area that has shown great success is the Family Resource Center (FRC) model. Planning began to address the concrete needs of families: Parent Advisory Council and Support groups (Education). Know and Grow grant awarded in 2024. Education was provided to FRC partners. FRC Network implemented: Gabriel's House, Safe Center, and Simmons Center to collaborate. These services may include, but are not limited to, connecting families and children to childcare, mental health services, housing, food pantries, diaper banks, personal and family finance education, and parent skill-building. The film Resilience and N.E.A.R. training were completed with all Family Resource Center staff. Parents were identified and met with to encourage participation in the Parent Advisory Committee through the FRC. Leaders attended the two-day PAC training. FRC staff and leadership also attended evidence-based Strengthening Families training through the Oklahoma Family Support Network, a department within the Oklahoma State Department of Health.$17,417$17,417

DRH Health was awarded an RHCORP Cancer Care Coordination grant for four years. Pathways is an active partner to address the priority of Cancer in the health improvement plan. The goal met was to organize a network of partners and increase partnerships/collaboration by three in FY 25. The Fall Back on Safety Event, also held annually, focuses on 4th graders and is in partnership with the Simmons Center. Multiple vendors promoted education on seasonal fall safety, including fire safety, Halloween, gun safety/wildlife and hunting, tobacco and vaping, youth substances, safe food preparation, and bicycle safety. Teachers evaluated the events and the resources presented to students as highly motivating and engaging. Vendors were evaluated on ease of registration, setup, and student participation, and feedback was exceptional.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt5Potts Know and Grow- Self-Healing Communities is an evidence-based model for identifying and addressing a community's needs and strengthening its core family structure. Pathways began learning about this model and needed work in 2019, and it has continued to gain momentum in rural areas of the state. One area that has shown great success is the Family Resource Center (FRC) model. Planning began to address the concrete needs of families: Parent Advisory Council and Support groups (Education). Know and Grow grant awarded in 2024. Education was provided to FRC partners. FRC Network implemented: Gabriel's House, Safe Center, and Simmons Center to collaborate. These services may include, but are not limited to, connecting families and children to childcare, mental health services, housing, food pantries, diaper banks, personal and family finance education, and parent skill-building. The film Resilience and N.E.A.R. training were completed with all Family Resource Center staff. Parents were identified and met with to encourage participation in the Parent Advisory Committee through the FRC. Leaders attended the two-day PAC training. FRC staff and leadership also attended evidence-based Strengthening Families training through the Oklahoma Family Support Network, a department within the Oklahoma State Department of Health.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt6Healthy Living- Partner with 3 area events to promote healthy living and address behavioral health in Stephens and Jefferson County. Activities included multiple 5K walk/runs; themed, family-friendly activities to encourage outdoor walking; and activities to educate about resources available to support a healthy, positive family structure. Increase awareness and education about the benefits of physical activity for school-aged children by identifying a teacher leader and offering pedometers to encourage walking during the day at school. Various health promotion events are held in both counties to promote overall health and wellness and positive mental health. Programming to address healthy nutrition utilizing the evidence-based Two-Bite club program with K and first-grade students. Garden Fresh is an evidence-based diabetes prevention and education program designed to prevent and reduce elevated A1C levels. Participation is ongoing, and referrals are made from a primary care provider to the program and monitored for a period of one year. DRH Health was awarded an RHCORP Cancer Care Coordination grant for four years. Pathways is an active partner to address the priority of Cancer in the health improvement plan. The goal met was to organize a network of partners and increase partnerships/collaboration by three in FY 25. Increased cancer screening opportunities were introduced to the communities, and DRH hired a CHW to coordinate additional care/needs for those requiring additional services.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt7Safety and Injury Prevention- The Spring into Safety Event, held annually at the local county fairgrounds, focuses on outdoor safety for 5th-grade students from Stephens and Jefferson counties. 12 vendors, including fire, weather, Halloween, substances, healthy relationships, hunting, gun, and lawn safety, participate. 250-300+ students attend the event each year. The Fall Back on Safety Event, also held annually, focuses on 4th graders and is in partnership with the Simmons Center. Multiple vendors promoted education on seasonal fall safety, including fire safety, Halloween, gun safety/wildlife and hunting, tobacco and vaping, youth substances, safe food preparation, and bicycle safety. Teachers evaluated the events and the resources presented to students as highly motivating and engaging. Vendors were evaluated on ease of registration, setup, and student participation, and feedback was exceptional.
