Gray Allison is Suzanne Grigsby's father but there are 5 other board members who are not related to the Grigsbys.Charlotte Miller, the treasurer for the ministry, is Suzanne Grigsby's sister. She is NOT a member of the board. She is a C.P.A.

Governing body meeting documentation Part VI line 8A

Suzanne Grigsby types minutes of the quarterly board meetings as the meeting is taking place. The minutes are reviewed by all the board members at the following board meeting. Any corrections or additions are made and then the board votes to approve the minutes, either as originally read, or as revised.

Form 990 governing body review Part VI line 11

The board approved the financial statements that were used to prepare FORM 990 and the audit of those financial statements was sustantially complete before the extended filing date of Form 990. A copy of the return will be reviewed by Jimmie Williams, a CFO of a company based in Memphis, TN, at the next board meeting in December, 2015.

Conflict of interest policy compliance Part VI line 12C

The board has discussed Conflict of Interest issues on an ongoing basis during meetings. A written Conflict of Interest policy was drawn up and approved at the December, 2011 board meeting. Each board member signed a statement stating that they agree with the policy and that they will notify the board immediately if any conflicts of interest occur in the future. The Conflict of Interest policy also stated that it will be reviewed annually at the meeting where the next year's budget is discussed (usually December) & each board member will annually sign the acknowledgement of agreement with the policy. The most review and signing was Dec. 6, 2014, when the board approved the budget for the 2015 year. The policy will be reviewed & discussed again in December, 2015, the next board meeting.

Other officer or key employee compensation Part VI line 15B

For 2014, there was a 4% overall raise for the Grigsbys and the Admin. salaries for the 2 part-time helpers to the ministry were previously raised $15 per hour in May, 2013 so no changes were made to administrative hourly rates for 2014.

Copies of Form 990 are available upon request. We have occasionally had a donor (or potential donor) request copies of the annual budget or income and expense information and have offered them a copy of the most recent Form 990 if they wish to review it but so far, no donors have wanted to see the Form 990...they have preferred a copy of the current year's budget or have merely requested more information about the board and operational policies.

Many other couples were given Biblical guidance by phone, in various cities around the country during retreats and on the way to and from retreats out of state, and in the Grigsby's home in Arlington, TN.

A copy of the return will be reviewed by Jimmie Williams, a CFO of a company based in Memphis, TN, at the next board meeting in December, 2015.

A written Conflict of Interest policy was drawn up and approved at the December, 2011 board meeting. Each board member signed a statement stating that they agree with the policy and that they will notify the board immediately if any conflicts of interest occur in the future. The Conflict of Interest policy also stated that it will be reviewed annually at the meeting where the next year's budget is discussed (usually December) & each board member will annually sign the acknowledgement of agreement with the policy. The most review and signing was Dec. 6, 2014, when the board approved the budget for the 2015 year. The policy will be reviewed & discussed again in December, 2015, the next board meeting.
