Chpa's 19 participating member organizations consist of federally-qualified health centers (fqhcs) which operate clinic sites and care for patients throughout colorado. Membership is voluntary.

Each member in the organization has equal voting rights and elects the directors to the board of directors through an annual election process, which is documented in the bylaws.

The board governs the day-to-day operations of the organization. The board of directors may alter the bylaws of the organization but if a bylaw change impacts or changes member's rights or privileges, the amendment requires an approval from the majority of the members. This has not occurred. The members of the organization also elect the directors to the governing body of the organization.

The board of directors discuss the information and vote to approve the form before the form is signed by the chief financial officer or the president of the board and then filed.

Community health provider alliance (chpa) has written and signed conflict of interest policies with all directors on the board. These are updated and signed annually by each director. Conflict of interest disclosures are required prior to and during every board meeting. This is monitored consistently by having conflict of interest disclosures as a standing agenda item for all board meetings. Board directors also participate in annual conflict of interest legal training.

Network ("ccmcm") effective january 1, 2025. Ccmcn is recognized as exempt from federal income tax under section 501 ( c )(3). The binding legal documents of the transaction describe the transaction as a merger. Ccmcm will be the surviving entity in the merger and chpa will cease to exist as a separate entity.

Program accomplishments include completion of performance year 5 (2024) of the contract period of the medicare shared savings program for 2020-2024. Shared savings for performance year 4 (2023) under track e of the mssp program totaled 17,288,693 of which 8,471,459 was received. Participating members received 4,052,719 in performance payments with an additional our mssp participating members. An additional 20% was allocated by the board of directors to fund member specific improvement efforts, 21% in for aco network improvement efforts along with 6% for the member development program and 5% to support chpa's operating costs. Chpa maintained and added to its portfolio of value-based aco contracts with medicare advantage and commercial insurance partners; development of population health strategies; continued business development of the aco.
