the executive director works with the cpa to provide data for the form 990. the completed draft is sent to the finance committee for review. the committee asks the cpa to make any needed changes. the finance committee then sends the form 990 to the entire board of directors for review.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

during the first meeting of each year, members of the board of directors are givenagain a copy of the conflicts of interest policy, and are asked to complete the disclosure form if there are any changes to their prior disclosure. a new board member always reviews and signs the disclosure and only updates it when necessary.
