The executive committee will then review the form 990 and related statements appropriately. If a member of the executive committee should have any questions or concerns, a teleconference will be scheduled to discuss these matters. If no questions or concerns arise, the executive director will sign the form 990 and inform the organizations cpa to file the return.”

Each responsible person shall annually complete a disclosure form identifying any relationships, positions or circumstances in which the responsible person is involved that he or she believes could contribute to a conflict of interest arising. Any persons with a conflict is prohibited from participating in the governing bodys deliberations and decisions in the transaction.”
