Form 990, Page 6, Part VI, Line 2

Kason hanson ashley hanson bod bod married

Form 990, Page 6, Part VI, Line 11B

The 990 is reviewed in detail by the finance committee and distributed to the full board for approval.

Form 990, Page 6, Part VI, Line 12C

Board members are required to review, sign, and commit to following the conflict of interest policy annually.

Form 990, Page 6, Part VI, Line 15A

The board of directors did not conduct a formal review for determining the compensation of the executive director. The compensation for the executive director remained at the same levels as in previous years. To ensure the compensation was reasonable, the executive director compiled a report using comparability data to demonstrate that the compensation was well below the industry average for similarly qualified persons in functionally comparable positions at similarly situated organizations. This data was presented to the board but did not fillow a formal review and approval process.

Form 990, Page 6, Part VI, Line 15B

The directors conducted a formal review process for determining the compensation of other key employees. This process included the following elements: review and approval: the compensation was reviewed and approved by the board of directors, excluding any individuals with a conflict of interest. Comparability data: the board used comparability data for similarly qualified persons in functionally comparable positions at similarly situated organizations. Documentation: contemporaneous documentation and recordkeeping were maintained with respect to the deliberations and decisions regarding the compensation arrangements. This formal review process was last undertaken for key employess during the previous tax year.

Form 990, Page 2, Part III, Line 4A

Ballet 5:8 employed 30 professional dancers and served more than 500 students annually through the school of ballet 5:8, providing training, mentorship, and career development for aspiring artists. The company reached thousands of audience members across illinois through live performances, partnerships, and educational outreach. Key initiatives included the world premiere of la llorona, a new ballet exploring postpartum depression and maternal mental health. The project was developed in partnership with the national museum of mexican art and the postpartum depression alliance of illinois, and included community workshops with the raices storytelling coalition, where latina mothers shared lived experiences to promote healing and awareness. Ballet 5:8 also forged a landmark collaboration with the kehrein center for the arts in chicagos austin neighborhood, bringing ballet to the venue for the first time. This performance celebrated the contributions of latina and black women in the arts and featured choreography by artistic director julianna rubio slager, guest artist rachel hutsell of new york city ballet, and spoken word artist quina aragon. Through these programs, ballet 5:8 expanded access to professional dance, amplified underrepresented voices, and strengthened illinoiss creative economy by employing artists and engaging communities statewide.

Financial Statement Notes

Schedule D, Page 3, Part X

The organization adopted accounting principles related to uncertain tax positions. Currently, the 2021, 2022 and 2023 tax years are open and subject to examination by the internal revenue service. However, the organization is not currently under audit, nor has the organization been contacted by this jurisdictions. The organization has evaluated its tax position taken for all open tax years. Based on the evaluation of the organization's tax positions, management believes all positions taken would be upheld under an examination. Therefore, no liabilty for the effects of uncertain tax positions was recorded at may 31, 2025. Any interst and penalties recognized and associated with a tax position would be classified as miscellaneous expense on the organization's financial statements. No such amounts were recognized in the year ended may 31, 2025.
