The form 990 is then presented to the full Board of Directors, reviewed, and recommended for approval at the next bi-monthly Board meeting before the Form 990 filing is due.”

“Form 990, Part VI, Section B, Line 12c - New Board Members are required to read and complete the Code of Conduct/Conflict of Interest Policy and Certification. Existing Board Members will periodically review the Code of Conduct/Conflict of Interest Policy and complete the Certification Forms if there are any changes to update the Board files. The Executive Director and Deputy Director monitors products and services provided to Southern Workforce Board, Inc. to ensure there is not a Conflict of Interest with staff or Board members by reviewing accounts payable and payroll before approving payments.”

“Form 990, Part VI, Section B, Line 15c - Human Resources does a wage comparability study with other workforce boards in the State of Oklahoma and the industry periodically for the Deputy Director, Fiscal Officer, Special Projects Manager, Quality Control Monitors, and the Accounting Clerk. The Administrative Committee, appointed by the Boards of Directors, review, evaluate and recommend salary increases for the Executive Director.”

“Form 990, Part VI, Section C, Line 19 - Southern Workforce Board, Inc. has all governing documents, conflicts of interest policy, and financial statements available to the public during normal working hours at the headquarter office upon written request.”

“Form 990, Part IX, Line 11g - Southern Workforce Board, Inc. (SWB) is funded by the U.S. Dept of Labor Workforce Innovative and Opportunity Act (WIOA) program that consists of 3 components. SWB performs 2 of the 3 components and sub-contracts with a third part to perform the 3rd component, which is a Servicer Provider. This line amount of $1,087,555 is the amount paid to the Service Provider to provide Career Management to clients/participants of the WIOA program.”
