IRS990ScheduleA/FactsAndCircumstancesTestTxt0Current Year Facts and Circumstances: The organization held annual fundraising dinners to solicit support from individual donors from the general public in 2013 and 2014. The organization submitted nine funding proposals to private foundations, and four funding proposals to federal, state, and municipal governments from 2011-2014. Our activities to solicit funds diminished in 2015 and 2016 as the organization considered new directions after the closing of our pilot grocery store in January 2015.The organization was financially supported by individual donations, and grant funding from the City of Chicago Small Business Improvement Fund, University of Chicago Medicine Community Benefit program, and the Consortium to Lower Obesity in Chicago Children (CLOCC).The organization was governed by a board comprised of local community members and leaders, and individuals with subject-matter expertise on the work of the organization.The organization operated a neighborhood grocery store from December 2012-January 2015, which benefited the immediate community (a "food desert") by providing a source of fresh fruits and vegetables where access had previously been lacking. The store also served the general public as a venue for nutrition education activities in partnership with nonprofit educators (e.g., University of Illinois Extension Nutrition Education Programs, EverThrive Illinois, Chef in the Hood), including informational in-store displays, "shopping healthy" store tours, and cooking classes in the store's demonstration kitchen. Finally, the store served the general public as a research laboratory that produced insights on food choice behaviors and mechanisms for incentvizing healthy food choice. From 2013-2016, the organization collaborated with researchers from the University of Chicago, University of Wisconsin-Madison, and University of California-San Francisco to conduct field-experiments to test incentives for healthy grocery shopping, and to analyze store sales data in order to draw insights that may inform public health policies.
