Senior charity care foundation's mission is to improve the quality of life for elders in need. Our purpose is to provide financial assistance for those in senior care settings and senior apartments who require care, services or support but have exhausted all other potential funding sources and do not have the financial resources to pay. We assist with basic health care needs not covered by medicare or medicaid (e.g. Dental care/dentures, hearing aids and eye glasses).

Here at senior charity care, we believe that every senior should have access to quality health care, regardless of income or insurance status. Our model focuses on affordable mobile health clinics. We bring dental and vision services directly to our clients.

Here at senior charity care, we believe that every senior should have access to quality health care, regardless of income or insurance status. Our model focuses on affordable mobile health clinics. We bring dental and vision services directly to our clients.

Form 990, Page 2, Part III, Line 4A

Formally established in 2011 as a utah-based 501c3 nonprofit organization, the mission of the senior charity care foundation (sccf) is to improve the quality of life for elders in need. Our purpose is to reduce healthcare disparities among seniors in poverty by addressing gaps in medicaid/medicare for dental care, hearing aids and prescription eyeglasses, and it is through our charity care program that we achieve our mission and purpose. For dental care/dentures, sccf has been operating mobile dental clinics since 2015. Sccfs mobile dental clinics removes the transportation, affordability and motivation/fear barriers by bringing high quality, affordable dental care to the senior residences. A dedicated team of licensed contracted dentists and sccf hygienist, specifically trained in evidence-based geriatric care, will travel to rose cove senior apartments (or one of the other sites) and provide a full-day clinic at least five times throughout the grant period. Dental services include the exam, x- rays, treatment planning, cleanings, extractions and fillings, semi- permanent crowns and dentures. Americorps members work directly with the seniors to complete the necessary documentation, make reminder calls, and collect outcome data and fees. For prescription eyeglasses, sccf operates our own mobile vision program. A licensed optometrist and opticians will visit the facilities/senior apartments to see up to twenty residents in one day. The residents will be prescreened and prequalified by our americorps members to ensure that they meet our program requirements and to obtain medical histories ensuring that they are ready to see the optometrist on the assigned day. Few seniors qualify for medicare coverage for their vision exams, but for those who do, they still cannot receive eyeglasses through traditional medicare and those with advantage plans; and, because they are in poverty and on fixed incomes, they find that the co-pays are cost prohibitive. Our optician will follow up with the seniors to make sure they select glasses that will work for them and to deliver them approximately two weeks later, ensuring their fit. We are asking each senior to pay 50 to cover the costs of the eyeglasses regardless of medicare coverage. For hearing aids, we partner with the starkey hear now foundation who collaborates with us to give the gift of hearing to seniors in need by providing access to high-quality hearing aids at an affordable price (250 per set). Retail costs for new hearing aids can be upwards to 10,000. When a senior applies for hearing aid assistance from sccf, one of our americorps members meets with the senior to help them through the application process with the starkey hearing foundation. The senior is asked to pay 20-30% of the hearing aid cost (50-75). It takes about a month for the starkey foundation to approve the application. Hearing aids are sent to the audiologist and the senior is contacted to make an appointment for the hearing aid fitting. Additional costs such as batteries and adjustments are the responsibility of the senior. Senior charity care foundation supports qualifying seniors on an ongoing, year-round basis, as funds are available. To qualify for assistance, a senior must be: age 55 (or older), uninsured or underinsured, reside in a skilled nursing, assisted living or hud-funded (section 8) or (below market rate rent) senior apartment; and, report an income at or below 200% (24,280 for a single person household and 32,920 for a two-person household) of the 2018 federal poverty level. Seniors can access our application on our website or by mail. Seniors typically are charged a nominal fee of 20-30% of the cost, which gives them dignity but we never send them to collections if they cannot afford to pay. Sccf payments for services rendered are sent directly to the care provider, not to the seniors themselves. We have grown from serving only two seniors in 2011 to serving 806 seniors in 2018. WHEN A SENIOR APPLIES FOR HEARING AID ASSISTANCE FROM SCCF, ONE OF OUR AMERICORPS MEMBERS MEETS WITH THE SENIOR TO HELP THEM THROUGH THE APPLICATION PROCESS WITH THE STARKEY HEARING FOUNDATION. THE SENIOR IS ASKED TO PAY 20-30% OF THE HEARING AID COST (50-75). IT TAKES ABOUT A MONTH FOR THE STARKEY FOUNDATION TO APPROVE THE APPLICATION. HEARING AIDS ARE SENT TO THE AUDIOLOGIST AND THE SENIOR IS CONTACTED TO MAKE AN APPOINTMENT FOR THE HEARING AID FITTING. ADDITIONAL COSTS SUCH AS BATTERIES AND ADJUSTMENTS ARE THE RESPONSIBILITY OF THE SENIOR. SENIOR CHARITY CARE FOUNDATION SUPPORTS QUALIFYING SENIORS ON AN ONGOING, YEAR-ROUND BASIS, AS FUNDS ARE AVAILABLE. TO QUALIFY FOR ASSISTANCE, A SENIOR MUST BE: AGE 55 (OR OLDER), UNINSURED OR UNDERINSURED, RESIDE IN A SKILLED NURSING, ASSISTED LIVING OR HUD-FUNDED (SECTION 8) OR (BELOW MARKET RATE RENT) SENIOR APARTMENT; AND, REPORT AN INCOME AT OR BELOW 200% (24,280 FOR A SINGLE PERSON HOUSEHOLD AND 32,920 FOR A TWO-PERSON HOUSEHOLD) OF THE 2018 FEDERAL POVERTY LEVEL. SENIORS CAN ACCESS OUR APPLICATION ON OUR WEBSITE OR BY MAIL. OUR PURPOSE IS TO REDUCE HEALTHCARE DISPARITIES AMONG SENIORS IN POVERTY BY ADDRESSING GAPS IN MEDICAID/MEDICARE FOR DENTAL CARE, HEARING AIDS AND PRESCRIPTION EYEGLASSES, AND IT IS THROUGH OUR CHARITY CARE PROGRAM THAT WE ACHIEVE OUR MISSION AND PURPOSE. FOR DENTAL CARE/DENTURES, SCCF HAS BEEN OPERATING MOBILE DENTAL CLINICS SINCE 2015. SCCFS MOBILE DENTAL CLINICS REMOVES THE TRANSPORTATION, AFFORDABILITY AND MOTIVATION/FEAR BARRIERS BY BRINGING HIGH QUALITY, AFFORDABLE DENTAL CARE TO THE SENIOR RESIDENCES. A DEDICATED TEAM OF LICENSED CONTRACTED DENTISTS AND SCCF HYGIENIST, SPECIFICALLY TRAINED IN EVIDENCE-BASED GERIATRIC CARE, WILL TRAVEL TO ROSE COVE SENIOR APARTMENTS (OR ONE OF THE OTHER SITES) AND PROVIDE A FULL-DAY CLINIC AT LEAST FIVE TIMES THROUGHOUT THE GRANT PERIOD. DENTAL SERVICES INCLUDE THE EXAM, X- RAYS, TREATMENT PLANNING, CLEANINGS, EXTRACTIONS AND FILLINGS, SEMI- PERMANENT CROWNS AND DENTURES. AMERICORPS MEMBERS WORK DIRECTLY WITH THE SENIORS TO COMPLETE THE NECESSARY DOCUMENTATION, MAKE REMINDER CALLS, AND COLLECT OUTCOME DATA AND FEES. FOR PRESCRIPTION EYEGLASSES, SCCF OPERATES OUR OWN MOBILE VISION PROGRAM. A LICENSED OPTOMETRIST AND OPTICIANS WILL VISIT THE FACILITIES/SENIOR APARTMENTS TO SEE UP TO TWENTY RESIDENTS IN ONE DAY. THE RESIDENTS WILL BE PRESCREENED AND PREQUALIFIED BY OUR AMERICORPS MEMBERS TO ENSURE THAT THEY MEET OUR PROGRAM REQUIREMENTS AND TO OBTAIN MEDICAL HISTORIES ENSURING THAT THEY ARE READY TO SEE THE OPTOMETRIST ON THE ASSIGNED DAY. FEW SENIORS QUALIFY FOR MEDICARE COVERAGE FOR THEIR VISION EXAMS, BUT FOR THOSE WHO DO, THEY STILL CANNOT RECEIVE EYEGLASSES THROUGH TRADITIONAL MEDICARE AND THOSE WITH ADVANTAGE PLANS; AND, BECAUSE THEY ARE IN POVERTY AND ON FIXED INCOMES, THEY FIND THAT THE CO-PAYS ARE COST PROHIBITIVE. OUR OPTICIAN WILL FOLLOW UP WITH THE SENIORS TO MAKE SURE THEY SELECT GLASSES THAT WILL WORK FOR THEM AND TO DELIVER THEM APPROXIMATELY TWO WEEKS LATER, ENSURING THEIR FIT. WE ARE ASKING EACH SENIOR TO PAY 50 TO COVER THE COSTS OF THE EYEGLASSES REGARDLESS OF MEDICARE COVERAGE. FOR HEARING AIDS, WE PARTNER WITH THE STARKEY HEAR NOW FOUNDATION WHO COLLABORATES WITH US TO GIVE THE GIFT OF HEARING TO SENIORS IN NEED BY PROVIDING ACCESS TO HIGH-QUALITY HEARING AIDS AT AN AFFORDABLE PRICE (250 PER SET). RETAIL COSTS FOR NEW HEARING AIDS CAN BE UPWARDS TO 10,000. WHEN A SENIOR APPLIES FOR HEARING AID ASSISTANCE FROM SCCF, ONE OF OUR AMERICORPS MEMBERS MEETS WITH THE SENIOR TO HELP THEM THROUGH THE APPLICATION PROCESS WITH THE STARKEY HEARING FOUNDATION. THE SENIOR IS ASKED TO PAY 20-30% OF THE HEARING AID COST (50-75). IT TAKES ABOUT A MONTH FOR THE STARKEY FOUNDATION TO APPROVE THE APPLICATION. HEARING AIDS ARE SENT TO THE AUDIOLOGIST AND THE SENIOR IS CONTACTED TO MAKE AN APPOINTMENT FOR THE HEARING AID FITTING. ADDITIONAL COSTS SUCH AS BATTERIES AND ADJUSTMENTS ARE THE RESPONSIBILITY OF THE SENIOR. SENIOR CHARITY CARE FOUNDATION SUPPORTS QUALIFYING SENIORS ON AN ONGOING, YEAR-ROUND BASIS, AS FUNDS ARE AVAILABLE. TO QUALIFY FOR ASSISTANCE, A SENIOR MUST BE: AGE 55 (OR OLDER), UNINSURED OR UNDERINSURED, RESIDE IN A SKILLED NURSING, ASSISTED LIVING OR HUD-FUNDED (SECTION 8) OR (BELOW MARKET RATE RENT) SENIOR APARTMENT; AND, REPORT AN INCOME AT OR BELOW 200% (24,280 FOR A SINGLE PERSON HOUSEHOLD AND 32,920 FOR A TWO-PERSON HOUSEHOLD) OF THE 2018 FEDERAL POVERTY LEVEL. SENIORS CAN ACCESS OUR APPLICATION ON OUR WEBSITE OR BY MAIL. SENIORS TYPICALLY ARE CHARGED A NOMINAL FEE OF 20-30% OF THE COST, WHICH GIVES THEM DIGNITY BUT WE NEVER SEND THEM TO COLLECTIONS IF THEY CANNOT AFFORD TO PAY.
