The board retains the services of an independent cpa firm to prepare the organization's form 990. Management reviews the completed form 990. A full copy of the form 990 is provided to all voting members of the governing body prior to filing. The governing body is provided a reasonable amount of time to review the return and ask any questions directly to organization management or the contact at the independent cpa firm prior to filing. Board members are also given the option to schedule a conference call to discuss the form 990 in greater detail if they have additional questions.

Conflicts of interest policy monitoring - officers, directors, trustees and key employees are annually required to report potential conflicts of interest. Potential conflicts are logged with and monitored by the secretary of the board.

Financial Statement Notes

PART X, LINE 2:

The foundation is a not-for-profit corporation and is exempt from tax under the provisions of internal revenue code section 501(c)(3). Accounting principles generally accepted in the united states of america require management to evaluate tax positions taken by the foundation and recognize a tax liability if the foundation has taken an uncertain position that more likely that not would not be sustained upon examination by the irs or other applicable taxing authorities. Management has analyzed the tax positions taken by the foundation and has concluded that as of december 31, 2013 and 2012, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the financial statements. The foundation is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress. Management believes it is no longer subject to income tax examinations for years prior to 2011.
