Working with youth that have been involved in a residential care facility and assisting them in transitioning back into the community and family. Providing supportive services designed to meet the need of the youth which may include educational enrichment opportunities, individual mentoring, youth and family case management service, resource and referral systems, resume preparation and job placement services, individual and family support meetings designed to remove barriers that will enable youth to be self supportive and lead productive lives.
