Northwest health services, inc contracted a chief operating officer from heartland regional medical center during a portion of fiscal year 2013-2014.

Form 990, Part VI, Section B, Question 11B

The form 990 is prepared by an independent accounting firm based on the audited financial statements and information provided by the accounting department of the organization. A tentative draft is initially reviewed by management. After this review, the finance committee then reviews the form 990 at their meeting before presenting it to the full board at their monthly meeting.

Form 990, Part VI, Section B, Question 12C

The organization regularly monitors compliance with their conflict of interest policy by requesting managers and staff to report any changes in their conflict of interest. Any conflicts are sent to the corporate compliance committee. If a conflict of interest is identified with a board member, they abstain from voting on any matters related to the conflict.

Form 990, Part VI, Section B, Question 15A & 15B

The ceo tasked the director of human resources to conduct a wage/salary survey for the purpose of updating the organization's salary/wage scale. Key concerns were internal equity and external competition. The survey was conducted over a three-month period using data from the medical group management association (mgma), compdata healthcare survey, national association of community health centers compensation survey, missouri primary care association compensation survey (using like demographics), and current organization salary/wage data. The ceo and all senior leadership salaries were part of this survey. Data was documented on an excel spreadsheet with proposed recommendations of the senior leadership. The salary/wage scale was then approved by the board, as noted in the board minutes. This process was last completed in august of 2012.
