Form 990, Part VI, Section B, Line 11B

The center's cpa firm prepares the form 990 in conjunction with the center's cfo. The cpa firm then prepares a final draft of the form 990 for review by the center's executive director and board of directors. After final review by the executive director and the board, the form 990 is filed with the internal revenue service.

Form 990, Part VI, Section B, Line 12C

The board president reviews the annual conflicts of interest filing by each of the board members. Any potential or real conflict of interest is resolved at that time. The cfo annually reports to the board on any items that could be a related party transaction. The board then determines the proper course of action with respect to any such related party transactions.

Form 990, Part VI, Section B, Line 15

Compensation process for top official the executive director/ceo's compensation is set by the full board based upon comparable compensation of similar executive director positions within the greater kansas city metro area. The qmrp's annual salary is set by the full board and is based upon the guidelines set forth by the missouri department of mental health. The cfo reports directly to the board; a sub-committee of the board votes to hire the cfo and set his compensation, which is based upon comparable fees charged by similar certified public accountants in the greater kansas city metro area. The board of directors receives no compensation for their board of director services. Form 990, part vi, line 15b - compensation process for officers there are no other officers within the organization. Compensation for other employees are determined by the executive director in consultation with the board of directors.

Form 990 Elections

Financial Statement Notes

PART X, LINE 2:

The center is exempt from federal income tax under section 501(c)(3) of the internal revenue code and a similar provision of state law and therefore has made no provision for income taxes in the accompanying financial statements; the center qualifies for the charitable contribution deduction under section 170(b)(1)(a) and has been classified as an center other than a private foundation under section 509(a). Income tax returns filed by the center on form 990 are subject to examination by the internal revenue service (irs) for three years after the date filed, with the result that fiscal years ending in 2018, 2017, 2016 and 2015 remain open as of may 15, 2019, although no returns have been selected for examination by the irs. Management has evaluated tax positions taken or expected in the course of preparing the center's tax returns with assistance from the center's tax preparer to determine whether its tax positions are more likely than not to be sustained by applicable taxing authorities; likely tax positions are reported in these financial statements, and management is not aware of any unlikely tax positions, with the result that no unlikely tax positions are reported.
