Governance Explanations

Form 990, Part VI, Section B, Line 11

The 990 tax return will be scanned and reviewed by the director of financial operations, who ensures the numbers align with the financial reports and discusses any necessary corrections with the auditing firm. The reviewed return is then emailed to all board members. The email will have the "read receipt" attached to it, so when the board member opens their email, the sender will be notified.

The executive director distributes a copy of the policy to board members upon membership and then again if any changes are made to the policy. The files are maintained by the executive director staff members sign a conflict of interest policy upon hire and then again if any changes are made to the policy. Copies are maintained in employee personnel records.

Compensation is determined based on an agency salary and wages table for each position. All positions have a 6 month introductory period, following which a person may be eligible to receive an increase if they have been successful in the performance of key functions identified in their job and subject to the availability of funds. This policy has been approved by the board of directors. Evalutaions were last completed in august 2014.

IT HAS BEEN REPLACED WITH A JOB PREPARATION SERVICE, WHICH IS JUST ONE OF THE MANY SERVICES CONSIDERED TO BE A PART OF THE CAREER SERVICES PROGRAM (PROGRAM #4). THE ULTIMATE GOAL OF THIS SERVICE, AND ALL EMPLOYMENT SERVICES AT ACT, IS PAID, INTEGRATED, COMMUNITY EMPLOYMENT. THIS SERVICE IS LIMITED TO 24 MONTHS AND IS DESIGNED TO PREPARE AN INDIVIDUAL TO SUCCEED IN COMMUNITY EMPLOYMENT. A PERSON'S SERVICES ARE DESIGNED TO TEACH HIM/HER THE GENERAL EMPLOYABILITY SKILLS NEEDED TO BE SUCCESSFUL (I.E. ATTENTION TO TASK, FOLLOWING DIRECTIONS, ACHIEVING PRODUCTIVITY STANDARDS, ATTENDANCE, PUNCTUALITY, APPROPRIATE ATTIRE, RESPONSE TO SUPERVISION, ACCEPTABLE WORK BEHAVIORS, ETC.). IN ADDITION TO TAKING PLACE AT ACT THROUGH TRAINING WITH THE RECYCLING OF PLASTIC, THE PROGRAM IS DIVERSIFYING THE LOCATION OF SERVICES TO ENSURE INDIVIDUALS ARE ABLE TO TRANSFER THEIR LEARNING TO MULTIPLE ENVIRONMENTS. DURING THE SECOND HALF OF FY 15, 19 INDIVIDUALS OBTAINED EMPLOYMENT, WHILE 12 WERE DISCHARGED FROM JOB DEVELOPMENT SERVICES PRIOR TO FINDING A JOB. 14 INDIVIDUALS WHO WERE ASSISTED WITH EMPLOYMENT WERE CLOSED SUCCESSFULLY.

THE PROGRAM IS A MULTI-ORGANIZATIONAL EFFORT INCLUDING ACT CAREER SERVICES, BOONE COUNTY FAMILY RESOURCES, BOONE HOSPITAL CENTER, THE CENTRAL MISSOURI REGIONAL OFFICE OF THE DIVISION OF DEVELOPMENTAL DISABILITIES, COLUMBIA PUBLIC SCHOOLS, AND VOCATIONAL REHABILITATION. IN FY15, SIX STUDENTS PARTICIPATED IN THE PROGRAM. THE STUDENTS RECEIVED JOB PREPARATION SERVICES PROVIDED BY ACT ALONG WITH SCHOOL CURRICULUM LED BY A CPS TEACHER. THEY SPENT SEVERAL MONTHS IN INTERNSHIP EXPERIENCES THROUGHOUT THE HOSPITAL. ALL SIX STUDENTS ALSO RECEIVED JOB DEVELOPMENT SERVICES FROM AN ACT CAREER SPECIALIST. ALL OF THE STUDENTS RECEIVED EMPLOYMENT OFFERS. AS OF JUNE 30, 2015, FIVE OF THE SIX STUDENTS WERE WORKING IN COMPETITIVE JOBS. THE OTHER STUDENT CONTINUED HIS JOB SEARCH, AS HE DECLINED HIS INITIAL JOB OFFER. THE INDIVIDUALS WHO OBTAINED EMPLOYMENT ALSO RECEIVED JOB SUPPORTS SERVICES FROM ACT CAREER SERVICES TO ASSIST WITH JOB RETENTION.
