The board of directors have entered into a contract with trinity health systems, inc., fort dodge, iowa, to provide certain management services, including ceo and other services, to the hospital.

The chief financial officer gathers information internally from various functional areas of the hospital. A draft copy of the return is provided to the chief financial officer for review. Financial Statement Notes

PART X, LINE 2:

The hospital is a nonprofit corporation under section 501(c)(3) of the internal revenue code and does not pay income tax on exempt-purpose income. Accounting principles generally accepted in the united states of america require management to evaluate tax positions taken by the hospital and recognize a tax liability (or asset) for an uncertain position that more likely than no would not be sustained upon examination by the internal revenue service. Management has evaluated their material tax positions and determined there are no uncertain positions taken or expected to be taken that would require recognition of a liability (or asset) or disclosure in the financial statements. The hospital is potentially subject to routine audits by tax authorities; however, there are currently no audits for any tax periods in progress. Management believes the hospital is no longer subject to income tax examinations for years ended prior to 2021.

THESE INCLUDE, BUT ARE NOT LIMITED TO GENERAL ACUTE CARE, SURGERIES, CRITICAL CARE, CARDIOLOGY, ONCOLOGY, REHABILITATION, LABORATORY SERVICES, PHARMACEUTICAL DRUGS, RADIOLOGY, AND MANY OTHER ROUTINE AND ANCILLARY SERVICES. SOME OF THE SERVICES PROVIDED DO NOT GENERATE ENOUGH INCOME TO OFFSET THEIR COST IN THE FISCAL PERIOD ENDED JUNE 30, 2025. THE HOSPITAL ADMITTED 105 PATIENTS RESULTING IN A TOTAL OF 354 PATIENT DAYS. OUTPATIENT REGISTRATIONS TOTALED 24,193. THERE WERE ALSO 2,267 EMERGENCY ROOM VISITS.
