Form 990, Part VI, Section B, Line 11

A draft of the 990 is given to each member of the audit committee for their review approximately one month before it is due. It is discussed in the regular monthly board meeting and any suggested changes are noted in the minutes of the meeting. These suggested changes are then made prior to filing the 990. The final form 990 is distributed to all board members via e-mail prior to filing with the irs.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is reviewed in the first board meeting of the year. Board members complete a form that documents all organizations with which they are affiliated. Any potential conflicts are identified. If any conflicts are identified, the board member is recused from participating in the conversation and vote pertaining to any related business transaction.

Form 990, Part VI, Section B, Line 15A

In july, the board of directors receives a copy of the executive director's goals and objectives for the upcoming year. In november, the board president sends a letter to the president of the resident council offering the opportunity for input by the council on the executive director's work. This letter requests a reply by the end of the year. In january or february the board of directors receives evaluation forms for the executive director, along with copies of the executive director's goals and objectives. The forms are returned to the board president. In march the executive board committee reviews input from the entire board, prepares a summary evaluation document and decides executive director compensation for budget preparation. In april the board president and executive director meet for a performance review discussion. The salary decision is shared with the executive director. The compensation decision is documented in writing and sent to chief financial officer for budget planning and human resources director for executive director's employee file. In may or june the executive director prepares updated goals and objectives for next fiscal year. The annual review of the executive director is based upon goals and objectives formulated through the strategic planning process.

Approximately 50 community volunteers provide recreational, spiritual, and social opportunities for our residents in assisted living and the health center.

FORM 990, PART XI, LINE 9:

Net income of affiliate - oaknoll foundation 32,056.

Financial Statement Notes

PART X, LINE 2:

The internal revenue service (irs)has deemed that christian retirement services, inc. And the foundation are exempt from federal income taxes under the provisions of section 501(c)(3). Oaknoll west is a disregarded entity for tax purposes. The organization has been granted partial exemption from real and personal property taxes on the retirement home facilities. Real estate and facilities associated with the skilled nursing unit are tax exempt. All other property is taxed in its entirety. All personal property of the organization is exempt from taxation. The organization collects property taxes from the residents on the basis of their apartment valuations, as determined by management. The organization is subject to federal and state income taxes to the extent it has unrelated business income. In accordance with the guidance for uncertainty in income taxes, management has evaluated their material tax positions and determined that there are no income tax effects with respect to its financial statements. The organization and the foundation are no longer subject to examination by the irs for years ending before june 30, 2012. The organization has not been notified of any impending examination by tax authorities and no examinations are currently in process.
