Serve low-income families and individuals living in the west metro community who are struggling to make ends meet each day.

Form 990, Page 2, Part III, Line 4A

Employment, housing, and computer literacy classes for individuals accessing our computers. 2. Community collaboration services: resourcewest hosts partner agencies onsite to provide additional support and outreach services. These services vary each year, but have included: free tax preparation, energy assistance applications, child and teen vaccination services, counseling services, employment services, senior food services, and/or dental care services. Mnsure navigator: in 2013-14, resourcewest received a one-year grant to hire a mnsure navigator that assisted individuals and families in applying for medical insurance. In 12 months, this service helped over 1,100 people connect and navigate mnsure, as well as processed over 57 medical applications. 3. Children and youth services: families with limited incomes can struggle to provide basic needs and additional expenses without breaking their budget. These expenses can also throw families into a financial crisis. Resourcewest offers programs that help families provide for their children and prevent financial crisis. -back to school: in august, over 1,000 school-age (k-12) children received new, grade-appropriate school supplies and a backpack. -winter warm wear: in october, over 800 children (age 0-18) received new or gently used winter jackets, boots, snow pants, hats, and mittens. -toy chest: in december, over 1000 children (age 0-18) received new holiday gifts. 4. Social services: resourcewest provides individualized support to high barrier individuals and families in crisis. Last year, nearly 600 individuals and families received social services support. -case management services: social workers provide short-term case management services including problem solving, counseling, advocacy, and resource connection that aim to alleviate the immediate needs of the family and provide support to stabilize the family long-term. Individuals and families in this program develop 1-3 month case plans that include goal setting, action planning, and follow-up. -financial assistance: car repairs, gas or bus cards, or prescriptions are provided to support individuals in maintaining their work or safety. -pilot project: in 2014, resourcewest began a partnership with a local faith community to provide long-term support to families at-risk of homelessness. The faith community provides a housing subsidy from 1-12 months provided that resourcewest provides case management assistance. This program has successfully served 4 families. 2013-14 statistics: -1,222 children in grades k-12 received school supplies and backpacks -772 children age 0-18 received warm winter wear -1,315 children age 0-18 received holiday gifts -10,225 resource connections -592 individuals and families participated in social services programs -25 families received car repairs or emergency gas or bus cards -624 individuals accessed free computers. -1,115 individuals were assisted in applying for medical insurance and 57 applications were processed.

COMMUNITY COLLABORATION SERVICES: RESOURCEWEST HOSTS PARTNER AGENCIES ONSITE TO PROVIDE ADDITIONAL SUPPORT AND OUTREACH SERVICES. THESE SERVICES VARY EACH YEAR, BUT HAVE INCLUDED: FREE TAX PREPARATION, ENERGY ASSISTANCE APPLICATIONS, CHILD AND TEEN VACCINATION SERVICES, COUNSELING SERVICES, EMPLOYMENT SERVICES, SENIOR FOOD SERVICES, AND/OR DENTAL CARE SERVICES. MNSURE NAVIGATOR: IN 2013-14, RESOURCEWEST RECEIVED A ONE-YEAR GRANT TO HIRE A MNSURE NAVIGATOR THAT ASSISTED INDIVIDUALS AND FAMILIES IN APPLYING FOR MEDICAL INSURANCE. IN 12 MONTHS, THIS SERVICE HELPED OVER 1,100 PEOPLE CONNECT AND NAVIGATE MNSURE, AS WELL AS PROCESSED OVER 57 MEDICAL APPLICATIONS. 3. CHILDREN AND YOUTH SERVICES: FAMILIES WITH LIMITED INCOMES CAN STRUGGLE TO PROVIDE BASIC NEEDS AND ADDITIONAL EXPENSES WITHOUT BREAKING THEIR BUDGET. THESE EXPENSES CAN ALSO THROW FAMILIES INTO A FINANCIAL CRISIS. RESOURCEWEST OFFERS PROGRAMS THAT HELP FAMILIES PROVIDE FOR THEIR CHILDREN AND PREVENT FINANCIAL CRISIS. -BACK TO SCHOOL: IN AUGUST, OVER 1,000 SCHOOL-AGE (K-12) CHILDREN RECEIVED NEW, GRADE-APPROPRIATE SCHOOL SUPPLIES AND A BACKPACK. -WINTER WARM WEAR: IN OCTOBER, OVER 800 CHILDREN (AGE 0-18) RECEIVED NEW OR GENTLY USED WINTER JACKETS, BOOTS, SNOW PANTS, HATS, AND MITTENS. -TOY CHEST: IN DECEMBER, OVER 1000 CHILDREN (AGE 0-18) RECEIVED NEW HOLIDAY GIFTS. 4. SOCIAL SERVICES: RESOURCEWEST PROVIDES INDIVIDUALIZED SUPPORT TO HIGH BARRIER INDIVIDUALS AND FAMILIES IN CRISIS. LAST YEAR, NEARLY 600 INDIVIDUALS AND FAMILIES RECEIVED SOCIAL SERVICES SUPPORT. -CASE MANAGEMENT SERVICES: SOCIAL WORKERS PROVIDE SHORT-TERM CASE MANAGEMENT SERVICES INCLUDING PROBLEM SOLVING, COUNSELING, ADVOCACY, AND RESOURCE CONNECTION THAT AIM TO ALLEVIATE THE IMMEDIATE NEEDS OF THE FAMILY AND PROVIDE SUPPORT TO STABILIZE THE FAMILY LONG-TERM. INDIVIDUALS AND FAMILIES IN THIS PROGRAM DEVELOP 1-3 MONTH CASE PLANS THAT INCLUDE GOAL SETTING, ACTION PLANNING, AND FOLLOW-UP. -FINANCIAL ASSISTANCE: CAR REPAIRS, GAS OR BUS CARDS, OR PRESCRIPTIONS ARE PROVIDED TO SUPPORT INDIVIDUALS IN MAINTAINING THEIR WORK OR SAFETY. -PILOT PROJECT: IN 2014, RESOURCEWEST BEGAN A PARTNERSHIP WITH A LOCAL FAITH COMMUNITY TO PROVIDE LONG-TERM SUPPORT TO FAMILIES AT-RISK OF HOMELESSNESS. THE FAITH COMMUNITY PROVIDES A HOUSING SUBSIDY FROM 1-12 MONTHS PROVIDED THAT RESOURCEWEST PROVIDES CASE MANAGEMENT ASSISTANCE.
