“The hpi board monitors potential conflicts of interest on the part of its board members, principal officers, members of committees with board delegated powers, and key employees ("covered persons") by maintaining a conflict of interest policy. Under the policy, covered persons annually are provided with a copy of the policy and asked to complete a questionnaire identifying any potential conflicts of interests. The general counsel of healthpartners reviews the questionnaire responses and develops a report detailing any potentially material conflicts for the president and chair of the board. A verbal summary is also given to the full board or appropriate committee ending with a reminder to covered persons of the policy's mandate that each person is obligated to disclose any new potential conflicts as they may arise throughout the year. Board agendas and executive decisions are monitored in relation to this policy. If a disclosed conflict of interest impacts an agenda item or decision, the covered person would be excluded from voting and may be excluded from receiving information and/or participating in deliberations, depending on the circumstances.”
“Hpi has no employees. All officers, directors and key employees are paid by group health, inc. ("ghi") or park nicollet health services (pnhs) a related organization. Ghi and pnhs have an annual process to review the market comparability of the total compensation of its ceo and its other key officers. Every two years, under the direction of the board of directors' compensation and leadership development committee (compensation committee), a total compensation market review of key officers is completed by an external compensation consultant. The review includes all components of compensation; base salary, annual incentives, benefits and perquisites. The market survey results are presented to, reviewed by and approved by the independent compensation committee. Based on this market data, the compensation committee determines minimum and maximum total compensation ranges for each key officer. In interim years, ghi's human resources staff, under the direction of the compensation committee, updates changes in the salary structure based on the same independent studies performed by the external compensation consultant for the compensation committee. For the chief executive officer and certain other positions full independent reviews are performed to set salary ranges based on the competitive market data specific to those positions. The compensation committee reviews and approves each year's key officer compensation results. In all cases, committee members complete an annual conflict of interest survey to assure the compensation committee members' independence and this is updated at any meeting at which decisions are being made. Staff (other than the secretary to the board) is not in the room during deliberations or vote including executive sessions, and contemporaneous minutes are kept. The board of directors has delegated to the executive committee the accountability to conduct an annual performance evaluation and to determine the compensation of the ceo based on the performance review and the market comparability data, approved by the compensation committee. The board has delegated to the ceo (with authority to further delegate) the accountability to conduct annual performance reviews and determine the compensation of all other officers within the compensation ranges determined by the compensation committee. Any exceptions to compensation in excess of the approved ranges are approved by the compensation committee. Total compensation is appropriately documented on the form 990 and w2 statements.”

Pediatric immunization incentive program. The pediatric immunization incentive program encourages pmap and mncare infants to get a series of vaccines by their second birthday. Once hpi receives the claims for these vaccines, members receive a $25 gift card in the mail. Hpi distributed 1,326 gift cards (valued at $35,139) for this program in 2024. The pre-teen (adolescent) immunizations incentive program. The pre-teen immunization program encourages pmap and mncare youth to get their hpv, meningococcal and tdap immunizations. Once hpi receives the claim for these vaccines, members receive a $25 gift card in the mail. Hpi distributed 1,212 gift cards (valued at $32,118) for this program in 2024. Postpartum incentive. Snbc, pmap and mncare members who receive a postpartum exam three to eight weeks after the birth of their child receive a $25 gift card. Hpi distributed 82 gift cards (valued at $2,193.50) for this program in 2024. Snbc incentives. To encourage an annual primary care visit, snbc members who complete their annual primary visit receive a $25 gift card. Members also receive a $25 gift card for doing a cervical cancer screening or completing a medication therapy management visit. Hpi distributed 472 gift cards (valued at $12,626) for these three programs in 2024.”
“Ed visit follow-up emergency department admissions for mental health and substance use conditions trigger telephonic outreach from our behavioral health case managers to members who are currently engaged in behavioral health case management or who have been identified for/engaged in behavioral health case management in the past but not engaged when hpi is notified of the emergency department visit. Upon notification of the visit, our case managers reach out to the members to assess what led to the visit, current gaps in or barriers to care and ongoing needs. If the member previously declined telephonic case management support, our case managers will attempt to engage and support these members in the aftermath of their emergency situation. From january-june 2024, 86 members were telephonically outreached in response to 93 emergency department visits for behavioral health concerns, as several members experienced more than one ed visit and received multiple outreaches. Of the 86 members who received this outreach, 41 were newly identified or re-identified for behavioral health case management because of their ed visit. The remaining members were already engaged in behavioral health case management and the contact they received in response to their ed visit was ongoing support they received as part of the service. From july-december, emergency department follow-up to members was paused as a result of the transition into a new case management documentation system. Suicide screening in 2018, the healthpartners institute (institute) was part of a large research study across seven health systems that developed and validated models to predict suicide attempts using health records and self-reported data. Based on the published research, in 2019, hpi replicated and implemented the model using health plan data to predict and identify members at rising risk of suicide. Upon identification, behavioral health case managers contact at-risk members in efforts to complete a suicide assessment and provide safety interventions to address risk. From october 2023-september 2024, 114 members who were engaged in behavioral health case management were identified as at-risk for suicide. Of these members, 64 (56%) were assessed for suicide risk using the columbia suicide severity screening. Based on the risk score and clinical picture of the members, case managers provided crisis intervention and/or safety planning and collaborated with providers to address known risk. The cost of this initiative, which is part of the behavioral health case management program and includes post-discharge support to help reduce admissions and readmissions and ed visit follow-up, was approximately $2.1m for salaries and benefits for behavioral health case managers. Improving patient safety and reducing medication & emergency room overuse and misuse hpi restricted recipient program provides case management support to members who demonstrate patterns of medication and health care overutilization and/or misuse. Following specific program protocols from the minnesota department of human services universal restricted recipient program, over 300 hpi members annually are restricted to a single physician, pharmacy and hospital to reduce unnecessary or harmful use of care while also improving continuity of care and health outcomes. The 2024 cost of the hpi's restricted recipient program was approximately $460,000 for salaries and benefits of restricted recipient case managers. Sustainability hpi's sustainability program is in alignment with and fully supports the organization's mission. The sustainability department is responsible for monitoring and making recommendations for reducing carbon emissions, minimizing waste, purchasing environmentally preferred materials when appropriate, minimizing our impact on the natural environment and interacting with the interested stakeholders of the organization and in the community. “In 2024, healthpartners community solar garden agreements produced the equivalent of nearly 865 houses' energy use for one year. In 2024, healthpartners also received over $479,000 in rebates by pursuing innovative energy efficiency projects. At the start of the pandemic, healthpartners teams quickly switched to video visits to keep both employees and patients safe. By embracing this new technology, patients who typically came to clinics for an in-person visit saved over 3.1 million miles and nearly 140,000 gallons of gas in 2024. Throughout remote working, employees traveled less for business purposes as well and saved $660,000 in mileage reimbursement costs in 2024 compared to pre-pandemic. Healthpartners won a 2024 system for change award for exemplary sustainability strategy for the eleventh year in a row from practice greenhealth, the leading health care sustainability organization in north america. Healthpartners has received additional high honors nationally from practice greenhealth, winning 17 awards in 2024. Additional awards healthpartners hospitals have received in 2024 include the tree campus healthcare from the arbor day foundation. Equitable care healthpartners collects data on race, ethnicity, language, and country of origin directly from patients and members in a variety of ways, all of them voluntary. These data collection sources include healthpartners.com, online through our health assessment, and the electronic medical record in our care delivery system and in our dental group. In addition, healthpartners receives data from the minnesota department of human services, minnesota community measurement, and minnesota immunization information connection (miic) for our members covered by minnesota health care programs. Healthpartners uses this data to monitor the quality of care delivered and patient/member experience by race, ethnicity and language. Healthpartners also uses the data to identify strategies to reduce health disparities in treatment, outcomes and service. Healthpartners includes race, ethnicity and language data on the member registries made available to network providers so they can identify and address disparities among their patients. Where data is voluntarily available on provider race, ethnicity, language and/or country of origin, healthpartners uses that data as a resource to respond to patient/member requests. Some of this data is available in the provider directories as well. Hospital equity alignment in 2024, healthpartners created the hospital system health care equity committee (hcec), which includes representatives from across healthpartners hospitals and meets bimonthly. In 2024, committee members discussed: - regulatory hospital standards from the minnesota hospital association, joint commission and centers for medicare & medicaid services; -key quality priorities, including falls prevention, hospital acquired pressure injuries and readmissions of patients with congestive heart failure and chronic obstructive pulmonary disease; - available health equity data and dashboards and system-wide work around social drivers of health. Local health equity committees continue to meet at methodist hospital, regions hospital and the valley (which includes representatives from lakeview, hudson, new richmond and amery hospitals) to share learnings from the hcec meetings and drive health care equity results at the local level. Health equity accreditation the national committee for quality assurance awarded hpi's medicaid plans its health equity accreditation status in 2023. The accreditation recognizes organizations that have successfully implemented market-leading practices to provide culturally and linguistically sensitive services and work to reduce health care disparities. Hpi holds the ncqa health equity accreditation status through 2025 and will go through the reaccreditation process in early 2026. The dhs civil rights complaint block contains information about member civil rights and how a member can file a complaint if they believe they have been discriminated against. The multi-language insert (mli) is a standalone document that must be included with all cms required materials for medicare individual and group plans. The mli is a standardized notification that informs the reader that interpreter services are available in 15 of the most common non-english languages in the united states. The affordable care act section 1557 non-discrimination notice and taglines are provided to all members. This occurs in our annually mandated information mailing in june. Created advertising on local radio stations and/or in diverse community publications for somali, hmong, native american, african american and hispanic populations. Development and promotion of unique supplemental benefits for msho members address social drivers of health and support members with chronic conditions, including benefits such as farmboxrx fresh produce delivery, animatronic support pets, caregiver support and tablets with education and wellness tools. Pregnant members in the healthy pregnancy program have access to personalized digital support through the my pregnancy experience when they log into healthpartners.com and associated outreach campaign. This includes personalized resources based on the pregnant person's race, ethnicity, language and insurance product. Similarly, members on a medicaid product receive personalized information about covered benefits such as covered doula support. Ongoing comprehensive, multi-channel campaign to support members and patients through medicaid redetermination. Direct outreach tactics included email, text, pre-recorded phone calls and web account prompts. Several were translated into spanish, hmong, somali, vietnamese, oromo, amharic and karen. In addition, we have broad awareness tactics including search-optimized web pages and blogs, community event participation, clinic-based flyers and signage, clinician and employer group outreach and more. Ongoing personalized communication approaches to close disparity gaps in colorectal cancer screening and cardiovascular medication adherence. Outreach for fit kits to screen for colorectal cancer incorporates messaging developed by the american cancer society's national colorectal cancer roundtable that has been proven to increase screening rates with black or african american members. The campaigns have resulted in an increase in screening rates both overall and within that member group. Additionally, an ongoing campaign encouraging cardiovascular medication adherence that features two trusted messengers from the black and african american community has resulted in a statistically significant increase in adherence by that member group. “Powerup in 2024, the powerup community health initiative continued to inspire and support kids and families to eat better, move more and feel good. This year, healthpartners deepened its focus on "feel good" resources, offering simple and approachable ways for kids to build healthy coping skills, confidence and emotional well-being. Whether it was through practicing mindfulness, finding moments to unplug, or spreading kindness, powerup provided fun and practical ideas to support kids in feeling their best. In-person outreach remained strong, connecting with families in meaningful ways through events, activities, and community partnerships. “- saint paul public library bookmobile: since 2014, healthpartners has sponsored the saint paul public library bookmobile, which is one of the last urban bookmobiles still in operation. The book mobile visits 45-50 sites on a regular basis in a two-week cycle, reaching about 120,000 people per year. The bookmobile ensures that the library is accessible to communities that face barriers in using the library's fixed facility due to transportation, cultural, language or other barriers. In addition to a financial contribution, healthpartners provides volunteers to help the bookmobile at local events.
