Mission

Provide person centered mental health and chemical health services to individuals and families in benton, sherburne, stearns and wright counties. We inspire hope and recovery, respond to the emerging needs of the community and support each other and our clients through an integrated approach to care.

Governance Explanations

Form 990, Part VI, Section A, Line 7A

The board of directors includes at least one county commissioner and lay person from each county served. The county commissioners and lay persons are selected by their respective counties.

Form 990, Part VI, Section A, Line 7B

All annual budget, including the operational subsidy and fee of service rates must be approved by the central minnesota board of directors and 3/4 of the human services boards of the four counties served. All major decisions must by approved the the foundation delegates.

The cfo reviews the irs form 990 and presents it to board of directors to review. After the final approval from the board of directors, the form 990 if signed and filed with the irs.

Form 990, Part VI, Section B, Line 12C

A conflict of interest or related party questionaire is provided annually to officers, directors and key employees before the first board meeting each year. The questionaire is completed by the board members and reviewed by the executive director and cfo. Executive committee and executive director review cases on an individual basis as they occur.

Form 990, Part VI, Section B, Line 15

The results of a compensation and benefits survey of comparable organinzations are compiled by the human resources department. The survey is broken down by position/job title. The executive director meets with all department supervisors to determine compensation changes based on performance, survey results and ratio analysis. The board of directors determines the total amount of compensaton increases based on budget projections. The survey results for the executive director are reviewed after the board of directors does a performance appraisal with the executive director. Based on the performance appraisal, the executive director salary is determined and reviewed to see if compensaton is reasonable for that position.

PART X, LINE 2:

U.s. Gaap requires management to evaluate tax positions taken by the organization and recognize a tax liability if the organization has taken an uncertain position that more likely than not would not be sustained upon examination by the internal revenue service. Management has analyzed the tax positions taken by the organization, and has concluded that as of december 31, 2016, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the financial statements. The organization is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress. The organization believes it is no longer subject to income tax examinations for years prior to 2013.

THE CENTER HAS A NUMBER OF PSYCHIATRISTS ON STAFF TO TREAT CHILDREN, ADOLESCENTS, AND ADULTS.

THIS PROGRAM IS OFTEN RECOMMENDED FOLLOWING PSYCHIATRIC HOSPITALIZATION OR, IF APPROPRIATE, AS AN ALTERNATIVE TO HOSPITALIZATION. DAY TREATMENT IS OFFERED IN THE ST. CLOUD AND BUFFALO LOCATIONS. THE PROGRAM IS 3 HOURS LONG EACH DAY.

CLIENTS MAY BE SELF-REFERRED OR REFERRED BY LAW ENFORCEMENT, HEALTH CARE PROVIDERS, SOCIAL SERVICES OR OTHER COMMUNITY AGENCIES. LENGTH OF STAY TYPICALLY RANGES FROM 12 TO 72 HOURS DURING WHICH TIME THE CLIENT IS MONITORED BY STAFF FOR SYMPTOMS OF WITHDRAWAL. CLIENTS MAY RECEIVE MEDICATION ACCORDING TO THE RECOMMENDATIONS OF THE MEDICAL DIRECTOR WHO IS AVAILABLE BY PHONE 24 HOURS A DAY.

IT IS HOUSED IN AN EIGHT-PLEX APARTMENT BUILDING IN WHICH FIFTEEN RESIDENTS SHARE FIVE APARTMENT UNITS. STAFF PROVIDE COVERAGE 24 HOURS A DAY. MEN AGES 18 OR OLDER WHO MEET THE CRITERIA FOR RULE 25 AND CONSOLIDATED TREATMENT FUNDING, WHO HAVE COMPLETED A PRIMARY CHEMICAL DEPENDENCY PROGRAM, AND HAVE A STRONG DESIRE TO MAINTAIN SOBRIETY ARE ELIGIBLE FOR ADMISSION ON A SPACE AVAILABLE BASIS.

BASED ON THE ALCOHOLICS ANONYMOUS 12-STEP APPROACH. THE PROGRAM TAKES ITS DIRECTION FROM A COMMUNITY BASED MENTAL HEALTH PROGRAM. THE CENTER HAS A TEAM OF LICENSED CHEMICAL DEPENDENCY COUNSELORS AND PSYCHOLOGISTS PROVIDING CARE. THE CENTER HAS A "FULL SERVICE" APPROACH. EACH PARTICIPANT IS TREATED AS AN INDIVIDUAL AND IS REVIEWED WEEKLY BY THE TREATMENT TEAM.
