The goal of the osceola community health foundation is to build healthy communities by fostering charitable support for the osceola medical center and the health-related needs of the people in the upper st. Croix valley.
The foundation works to identify needs and provide health related services to residents of our community. Using philanthropic assistance, we strive to support the medical center and implement health related programs that benefit all age groups.

THIS HELPED OMC COVER SOME COSTS FOR THE CONSULTING OF SPACE PLANNING. WE ESTIMATE 5 MEETINGS WERE HELD AND CONSULTANT DRAWINGS WERE COMPLETED THAT THIS GRANT HELPED SUPPORT. PATIENT CARE FUND- THE FOUNDATION PROVIDES CLOTHING AND GAS/FOOD COUPONS FOR PATIENTS WITH EMERGENT NEEDS FOR ANY AND ALL OF THESE. WE ESTIMATE 40 PATIENTS WERE HELPED THROUGH THIS PROGRAM. STAFF IS ABLE TO GRAB CLOTHING FOR PATIENTS OF ALL AGES AS NEEDED FROM A SUPPLY THE FOUNDATION PROVIDES. GAS/FOOD COUPONS ARE AVAILABLE IN BINDERS LOCATED THROUGHOUT THE FACILITY AND TRACKED VIA OUR QUALITY IMPROVEMENT OUTCOME SYSTEM. 10 PATIENTS WERE SUPPORTED WITH GAS/FOOD COUPONS IN 2024. THIS PORTION OF THE PROGRAM WAS NOT INITIATED UNTIL MID 2024. EXTERNAL COMMUNITY GRANTS- WE SUPPORTED 2 COMMUNITY ORGANIZATIONS WHICH ALIGN WITH THE MISSION OF OMC TO IMPROVE THE HEALTH AND WELLBEING OF THE PEOPLE IN THE UPPER ST. CROIX VALLEY. THE FIRST ORGANIZATION'S MISSION IS TO PROVIDED FAMILY EDUCATION AND PARENTING SKILLS FOR NEW PARENTS IN THE RURAL AREAS OF OUR MARKET, INCLUDING THOSE WHO ARE NON-ENGLISH SPEAKING. THEIR MISSION IS TO HELP IMPROVE SAFETY AND HEALTH IN FAMILY'S HOMES. THEY WERE ABLE TO DOUBLE THE NUMBER OF FAMILIES SERVED TO 28 AND DOUBLE THE NUMBER OF VISITS FOR THESE CLIENTS TO 119. THE NEXT ORGANIZATION PROVIDES TRANSITIONAL HOUSING FOR WOMEN AFTER IMPRISONMENT FOR UP TO A YEAR AND IS SPECIFICALLY FOR THOSE WHO HAVE RECOVERED FROM SUBSTANCE ABUSE AND NEED FURTHER SUPPORT UPON RELEASE TO CONTINUE IMPROVING AND RESTART LIFE SOBER. THIS ORGANIZATION WAS ABLE TO HELP 16 WOMEN IN 2024 TO GAIN EMPLOYMENT, DRIVER'S LICENSE, AND ACCESS TO CHILDREN. |
