Mission

The mission of woodland hospice is to maintain a residential facility to support high quality, dignified, compassionate, end-of-life care to people with terminal illness; to provide financial assistance to uninsured and under-insured hospice patients requiring end-of-life care in our residential hospice facility, and to sponsor programs providing bereavement care to the community.

Form 990, Part III, Line 3

During September 2015, the Organization entered into an operations transfer agreement with MidMichigan Visiting Nurse Association, whereby MidMichigan would provide hospice care services for patients on behalf of the Organization. This agreement allowed for the transfer of care to begin effective immediately upon the signing of the agreement, and outlined that a future closing date would be established once the Medicaid license was transferred from the Organization to MidMichigan. Woodland Hospice House, an 8 bed facility, would continue to be owned and maintained by the Organization as a residential hospice facility.
