The mission of hospice at home is to serve our patients, families and communities with dedication and compassion; to deliver the best end of life care to help people experience peaceful, pain-free and sacred deaths within the context of their own lives; and to guide our youth, adults and families through the grief process with timely and consistent availabilty.

The organization is organized on a membership basis. The sole member of the organization is lakeland regional health system, a michigan not-for-profit corporation.

Form 990, Part VI, Line 7A Members or stockholders electing members of governing body

As the sole member of the organization, lakeland regional health system has the power to appoint new board members to the hospice at home governing body.

Form 990, Part VI, Line 7B Decisions requiring approval by members or stockholders

As the sole member of the organization, lakeland regional health system has the authority to: 1. Select and remove the chief executive officer, which will be done in collaboration with the board of the corporation; 2. Approve the creation or modification of indebtedness (including guarantees) other than trade payables, whether secured or unsecured, in excess of $100,000; 3. Approve leases and other agreements which are either (i) not in the ordinary course of business or (ii) exceed $50,000 and have not already been approved in either the capital or operating budgets; 4. Approve annual capital and operating budgets following collaboration with the board of the corporation and its leadership. Both this corporation and lakeland will keep in mind what is in the best interests of both lakeland and this corporation, and will be consistent with required lakeland system consolidated targets; 5. Approve any sale, disposal or other transfer of assets over $50,000; 6. Approve any individual non-budgeted expenditure exceeding $50,000; 7. Approve amendments to the governing documents of the corporation; and 8. Remove corporation board members, which shall only be for cause.
