Governance Explanations

Form 990, Part VI, Section B, line 11B

Process of review of form 990 by the board: the treasurer prepares the form 990 from the accounting records and gives a copy to each board member at the board meeting. The treasurer then goes through each page with the board and discussion ensues. The applicable parts of the return are compared to the financial statements previously approved by the board and differences, if any, are reconciled. All other applicable parts of the return are matched up to the board's stated policies and transactions during the year. Each board member then has an opportunity to further their review of the return after the meeting and if any further questions or comments exist, they email the treasurer by a certain date. After this occurs, the return is filed timely.

The board has previously adopted a conflict of interest policy. No conflict of interest has occurred during the year. As stated below, some board members have family residing in the facility. However, all residents are treated the same and follow the specific rules of admission and procedures as outlined in the by laws and the standard approved life lease contract. This is always without exception. A few directors have family members as residence of apostolic christian prairie villa.

A notice in the office of the facility states the availability of these. A copy of the financial statements and tax return is available in the accounting office.

Part X, Line 2:

Apostolic christian prairie villa, inc. Continues to be organized and operates as a tax-exempt organization, meeting "the more likely than not" threshold consistent with its exempt status. No change has been made on operations since inception of prairie villa. The mission remains the same and approximately 99% of income is derived from this tax exempt mission. The remaining 1% is derived from interest received from cash in a c/d at the bank held for mission purposes. Therefore, prairie villa financial statements recognizes no liability or provision for income taxes.
