Form 990, Part III, Line 3 Significant changes in program services

On august 19, 2022, the association's board of managers (board) approved the termination the plan effective december 31, 2022. As part of the plan's termination, the board amended the plan to accept health care benefit claims for any benefits incurred prior to december 31, 2022 through march 31, 2023. Thereafter, the plan accepted no further claims. The association completed its financial affairs by june 30, 2023 and placed all remaining assets in escrow at first merchants bank through december 31, 2029, or such date as may be established by the board (the escrow period).
