In 2023, The Vitality Project (TVP) took on the role of lead agency for the Tehama County Continuum of Care (Tehama CoC), a collaborative of local service providers and community members whose objective is to prevent and end homlessness in rural Tehama County California. TVPs role with Tehama CoC include coordinating collaborative processes, including strategic planning, gaps analysis, ensuring equitable access to services, serving as a hub for local distribution of federal, state, and local funds for shelter and housing programs, and maintaining the community-wide Homeless Managament Information System (HMIS), a shared database into which service projects supported by a number of state and federal enter service data.

Cessation of or significant change to any program service Part III line 3

Fiscal and administrative support for the annual Live Inspired for Tomorrow (LIFT) Event, a collaboratively planned, coordinated, and staffed event at which Tehama County residents can connect with, and in some cases, directly access a wide variety of resources, including medical and dental, public benefit, Social Security, DMV IDs, education, job training and employment, clothing, food, and other basic needs items.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt3In 2023, The Vitality Project (TVP) took on the role of lead agency for the Tehama County Continuum of Care (Tehama CoC), a collaborative of local service providers and community members whose objective is to prevent and end homlessness in rural Tehama County California. TVPs role with Tehama CoC include coordinating collaborative processes, including strategic planning, gaps analysis, ensuring equitable access to services, serving as a hub for local distribution of federal, state, and local funds for shelter and housing programs, and maintaining the community-wide Homeless Managament Information System (HMIS), a shared database into which service projects supported by a number of state and federal enter service data.
