Mission

Transitions foundation supports the mental health and rehabilitation programs and services provided by transitions of western illinois, a not- for-profit corporation located in quincy, il. The foundation also supports the adams county mental health and retardation association doing business as the transitions association, a not-for-profit corporation that owns the real estate and property that is utilized in the delivery of transitions' services. Transitions' mission is to provide human services that inspire growth and improve the quality of life for the community by empowering people of all abilities to lead full and meaningful lives. Services are available to residents of adams county in illinois, with some services available to residents of other surrounding illinois counties. Transitions was founded in 1955 as the adams county mental health center and for many years was known as the community counseling center. In 1995 the agency changed its name to transitions of western illinois.

Transitions foundation supports the programs and services of transitions of western illinois and the adams county mental health and retardation association, charitable, not-for-profit organizations located in quincy, il.

Transitions foundation supports the mental health and rehabilitation programs and services provided by transitions of western illinois, a not- for-profit corporation located in quincy, il. The foundation also supports the adams county mental health and retardation association doing business as the transitions association, a not-for-profit corporation that owns the real estate and property that is utilized in the delivery of transitions' services. Transitions' mission is to provide human services that inspire growth and improve the quality of life for the community by empowering people of all abilities to lead full and meaningful lives. Services are available to residents of adams county in illinois, with some services available to residents of other surrounding illinois counties. Transitions was founded in 1955 as the adams county mental health center and for many years was known as the community counseling center. In 1995 the agency changed its name to transitions of western illinois. For over 60 years transitions has served the adams county area, providing high quality, affordable mental health and rehabilitation services. Since 1977 transitions has been accredited by carf (commission for the accreditation of rehabilitation facilities). In addition, the agency is a certified medicaid provider and is licensed by the illinois department of human services. Transitions helps people of all ages, from infants and toddlers to senior citizens. The agency also serves people of all abilities, people who are disabled due to a severe developmental disability to people who are not disabled, but who are struggling with a life problem that could affect anyone (grief and loss, job stress, interpersonal problems). Transitions has programs and services to help: -young children who are delayed in their development or at risk of having such delays by providing developmental services and parent education. -youth and adults who are struggling with emotional adjustment problems by providing outpatient counseling, crisis intervention services, and substance use disorder treatment. -adults with a serious mental illness by providing supportive counseling services and educational services to help them manage their illnesses. -adults who have a disability and/or are economically disadvantaged learn skills that will enable them to get and keep a job. -adults with a mental disability learn skills of daily living ( i.e., grooming skills, how to dress for work, how to tell time, how to use the city bus system, how to budget, etc.). -children who have very severe mental and physical disabilities and need full time care by providing special education services at the transitions school. The school not only provides educational services to the children, it provides respite to students' parents, who otherwise have the responsibility for full-time care of their child with a severe disability. -adults who have a serious mental illness or developmental disability live successfully in the community. Transitions provides 24-hour supervision and support to residents living in group homes, and intermittent support to residents living in apartments in the community. -children whose parents are in the process of divorce by providing an educational program to help parents focus on the needs of their children during divorce. Annually, transitions serves more than 9,000 residents of adams county and west central illinois. Gifts to the transitions foundation help support transitions' services, enabling the agency to provide services to people who lack the means to pay the full cost of the services they receive. Thanks to gifts from individuals, businesses, and foundations, transitions' services are accessible to people in our area, regardless of income.

The endowment fund within transitions foundation further supports transitions' mission by providing funding to support program services. Often, grants and fee-for-service funding does not pay the cost of services. Endowment funds supplement this funding and therefore, support services. For example, endowment funds assist with providing affordable mental health care to residents who do not have medicaid or insurance funding; they assist in supporting residential services to adults who have a developmental disability as state funding does not cover the actual costs of providing these services; they assist with supporting special education services to children who have severe developmental and physical disabilities who attend the transitions school as state board of education funding does not cover all the costs of providing these educational services. Endowment funds also assist with the purchase of supplies and equipment needed in the delivery of mental health, rehabilitation, and educational services.

TRANSITIONS' MISSION IS TO PROVIDE HUMAN SERVICES THAT INSPIRE GROWTH AND IMPROVE THE QUALITY OF LIFE FOR THE COMMUNITY BY EMPOWERING PEOPLE OF ALL ABILITIES TO LEAD FULL AND MEANINGFUL LIVES. SERVICES ARE AVAILABLE TO RESIDENTS OF ADAMS COUNTY IN ILLINOIS, WITH SOME SERVICES AVAILABLE TO RESIDENTS OF OTHER SURROUNDING ILLINOIS COUNTIES. TRANSITIONS WAS FOUNDED IN 1955 AS THE ADAMS COUNTY MENTAL HEALTH CENTER AND FOR MANY YEARS WAS KNOWN AS THE COMMUNITY COUNSELING CENTER. IN 1995 THE AGENCY CHANGED ITS NAME TO TRANSITIONS OF WESTERN ILLINOIS.

A GRANT SELECTION COMMITTEE, CONSISTING OF THE PRESIDENT, TREASURER, AND DIRECTOR OF DEVELOPMENT, REVIEW THE REQUESTS AND DETERMINE WHICH PROPOSALS WILL BE FUNDED AND THE AMOUNT OF FUNDING THAT WILL BE PROVIDED. PROGRAM GRANTEES ARE EXPECTED TO COMPLETE A REPORT OF GRANT FUNDED ACTIVITIES AND EXPENDITURES AND SUBMIT THIS REPORT TO THE DIRECTOR OF DEVELOPMENT AT THE END OF THE GRANT PERIOD. THE DIRECTOR OF DEVELOPMENT REPORTS GRANT FUNDED ACTIVTIES AND EXPENDITURES TO THE FOUNDATION BOARD AT THEIR REGULARLY SCHEDULE MEETINGS. THE FOUNDATION'S RFP PROCESS HAS PROVIDED FUNDING FOR RECREATIONAL OPPORTUNITIES FOR CONSUMERS OF TRANSTIONS' SERVICES. GRANTS HAVE BEEN REQUESTED TO TAKE RESIDENTS OF A GROUP HOME TO A MAJOR LEAGUE BASEBALL GAME, TO THE ILLINOIS STATE FAIR, AND TO ST. LOUIS TO VISIT SUCH SITES AS THE ZOO, SCIENCE CENTER, AND GRANT'S FARM. GRANTS HAVE BEEN REQUESTED TO TAKE CHILDREN WITH SEVERE DEVELOPMENTAL DISABILITIES TO A MOVIE AT THE LOCAL THEATRE. GRANTS HAVE ENABLED ADULTS WITH A SERIOUS MENTAL ILLNESS TO RIDE THE AMTRACK TO CHICAGO AND VISIT THE SHEDD AQUARIUM. OUTINGS THAT MOST OF US JUST TAKE FOR GRANTED ARE VERY SPECIAL TO THE PEOPLE WHO RECEIVED TRANSITIONS' SERVICES. MANY HAVE NEVER TRAVELED OUTSIDE OF QUINCY AND MOST DO NOT HAVE THE FINANCIAL RESOURCES OR SUPPORTS TO DO SO. THE RECREATIONAL OUTINGS PROVIDED BY TRANSITIONS FOUNDATION GRANTS ARE VERY MUCH APPRECIATED BY THOSE TRANSITIONS SERVES. THESE OUTINGS PROVIDE OPPORTUNITIES FOR SKILL DEVELOPMENT AND LEARNING AND GREATLY IMPROVE THE QUALITY OF CONSUMERS' LIVES.

THE FOUNDATION ALSO SUPPORTS THE ADAMS COUNTY MENTAL HEALTH AND RETARDATION ASSOCIATION DOING BUSINESS AS THE TRANSITIONS ASSOCIATION, A NOT-FOR-PROFIT CORPORATION THAT OWNS THE REAL ESTATE AND PROPERTY THAT IS UTILIZED IN THE DELIVERY OF TRANSITIONS' SERVICES. TRANSITIONS' MISSION IS TO PROVIDE HUMAN SERVICES THAT INSPIRE GROWTH AND IMPROVE THE QUALITY OF LIFE FOR THE COMMUNITY BY EMPOWERING PEOPLE OF ALL ABILITIES TO LEAD FULL AND MEANINGFUL LIVES. SERVICES ARE AVAILABLE TO RESIDENTS OF ADAMS COUNTY IN ILLINOIS, WITH SOME SERVICES AVAILABLE TO RESIDENTS OF OTHER SURROUNDING ILLINOIS COUNTIES. TRANSITIONS WAS FOUNDED IN 1955 AS THE ADAMS COUNTY MENTAL HEALTH CENTER AND FOR MANY YEARS WAS KNOWN AS THE COMMUNITY COUNSELING CENTER. IN 1995 THE AGENCY CHANGED ITS NAME TO TRANSITIONS OF WESTERN ILLINOIS. FOR OVER 60 YEARS TRANSITIONS HAS SERVED THE ADAMS COUNTY AREA, PROVIDING HIGH QUALITY, AFFORDABLE MENTAL HEALTH AND REHABILITATION SERVICES. SINCE 1977 TRANSITIONS HAS BEEN ACCREDITED BY CARF (COMMISSION FOR THE ACCREDITATION OF REHABILITATION FACILITIES). IN ADDITION, THE AGENCY IS A CERTIFIED MEDICAID PROVIDER AND IS LICENSED BY THE ILLINOIS DEPARTMENT OF HUMAN SERVICES. TRANSITIONS HELPS PEOPLE OF ALL AGES, FROM INFANTS AND TODDLERS TO SENIOR CITIZENS. THE AGENCY ALSO SERVES PEOPLE OF ALL ABILITIES, PEOPLE WHO ARE DISABLED DUE TO A SEVERE DEVELOPMENTAL DISABILITY TO PEOPLE WHO ARE NOT DISABLED, BUT WHO ARE STRUGGLING WITH A LIFE PROBLEM THAT COULD AFFECT ANYONE (GRIEF AND LOSS, JOB STRESS, INTERPERSONAL PROBLEMS). TRANSITIONS HAS PROGRAMS AND SERVICES TO HELP: -YOUNG CHILDREN WHO ARE DELAYED IN THEIR DEVELOPMENT OR AT RISK OF HAVING SUCH DELAYS BY PROVIDING DEVELOPMENTAL SERVICES AND PARENT EDUCATION. -YOUTH AND ADULTS WHO ARE STRUGGLING WITH EMOTIONAL ADJUSTMENT PROBLEMS BY PROVIDING OUTPATIENT COUNSELING, CRISIS INTERVENTION SERVICES, AND SUBSTANCE USE DISORDER TREATMENT. -ADULTS WITH A SERIOUS MENTAL ILLNESS BY PROVIDING SUPPORTIVE COUNSELING SERVICES AND EDUCATIONAL SERVICES TO HELP THEM MANAGE THEIR ILLNESSES. -ADULTS WHO HAVE A DISABILITY AND/OR ARE ECONOMICALLY DISADVANTAGED LEARN SKILLS THAT WILL ENABLE THEM TO GET AND KEEP A JOB. -ADULTS WITH A MENTAL DISABILITY LEARN SKILLS OF DAILY LIVING ( I.E., GROOMING SKILLS, HOW TO DRESS FOR WORK, HOW TO TELL TIME, HOW TO USE THE CITY BUS SYSTEM, HOW TO BUDGET, ETC.). -CHILDREN WHO HAVE VERY SEVERE MENTAL AND PHYSICAL DISABILITIES AND NEED FULL TIME CARE BY PROVIDING SPECIAL EDUCATION SERVICES AT THE TRANSITIONS SCHOOL. THE SCHOOL NOT ONLY PROVIDES EDUCATIONAL SERVICES TO THE CHILDREN, IT PROVIDES RESPITE TO STUDENTS' PARENTS, WHO OTHERWISE HAVE THE RESPONSIBILITY FOR FULL-TIME CARE OF THEIR CHILD WITH A SEVERE DISABILITY. -ADULTS WHO HAVE A SERIOUS MENTAL ILLNESS OR DEVELOPMENTAL DISABILITY LIVE SUCCESSFULLY IN THE COMMUNITY. TRANSITIONS PROVIDES 24-HOUR SUPERVISION AND SUPPORT TO RESIDENTS LIVING IN GROUP HOMES, AND INTERMITTENT SUPPORT TO RESIDENTS LIVING IN APARTMENTS IN THE COMMUNITY. -CHILDREN WHOSE PARENTS ARE IN THE PROCESS OF DIVORCE BY PROVIDING AN EDUCATIONAL PROGRAM TO HELP PARENTS FOCUS ON THE NEEDS OF THEIR CHILDREN DURING DIVORCE. ANNUALLY, TRANSITIONS SERVES MORE THAN 9,000 RESIDENTS OF ADAMS COUNTY AND WEST CENTRAL ILLINOIS. GIFTS TO THE TRANSITIONS FOUNDATION HELP SUPPORT TRANSITIONS' SERVICES, ENABLING THE AGENCY TO PROVIDE SERVICES TO PEOPLE WHO LACK THE MEANS TO PAY THE FULL COST OF THE SERVICES THEY RECEIVE.
