“The Foundation's process for preparation and review of this Form 990 involves the following steps: The organization's accountant works closely with Advocate tax personnel and the Executive Director to prepare an initial draft of the return. The organization's accountant, who also prepares the monthly financial statements, is familiar with the organization and has experience with tax-exempt return preparation. A draft of the return was then reviewed in detail by the Foundation's outside tax consulting firm who followed up on any unusual or significant matters. The preliminary Form 990 is then reviewed by the Executive Director, the Chairman and other select Board members prior to filing. A completed copy of this Form 990 is made available to all Board members.”
