Number of individuals employed in the calendar year - Five individuals worked for the Organization during the calendar year. These individuals were paid by Accreditation Association for Ambulatory Health Care, Inc.(AAAHC)(FEIN 36-3016881), a related tax exempt organization. AAAHC-IQI reimburses AAACH for all salaries, benefits and related taxes.

Part X, Line 2:

FIN 48 Note from Audited Financial Statements AAAHC - IQI is exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code and, therefore, have made no provision for federal income taxes in the accompanying financial statements. The tax exempt purpose of AAAHC - IQI and the nature in which it operates is described in the first paragraph of Note 1. AAAHC - IQI continues to operate in compliance with its tax exempt purpose and management's assessment is there are no uncertain tax positions. The annual information and income tax returns filed with the federal and state governments are subject to examination for for years ended June 30, 2012 through June 30, 2015.

The tax exempt purpose of AAAHC - IQI and the nature in which it operates is described in the first paragraph of Note 1. AAAHC - IQI continues to operate in compliance with its tax exempt purpose and management's assessment is there are no uncertain tax positions. The annual information and income tax returns filed with the federal and state governments are subject to examination for for years ended June 30, 2012 through June 30, 2015.
