Financial Statement Notes

PART V, LINE 4:

To provide support for the programs and mission of wellness house.

PART X, LINE 2:

Wellness house has been determined to be exempt from income tax under section 501(c)(3) of the internal revenue code as a not-for-profit organization, and accordingly, no provision has been made for either federal or state income taxes. Wellness house has evaluated their tax positions taken for all open tax years. Currently, the 2017, 2018 and 2019 tax years are open and subject to examination by the internal revenue service, however, wellness house is not currently under audit and has not been contacted by this jurisdiction. Based on the evaluation of the wellness house's tax positions, management believes all positions taken would be upheld under an examination; therefore, no provision for the effects of uncertain tax positions has been recorded for the years ended june 30, 2021 and 2020.

INDIVIDUALS HAVE MADE 26,162 VISITS TO OUR EXERCISE AND NUTRITION PROGRAMS DURING THE YEAR. ALL PROGRAMMING WAS MOVED TO ONLINE PLATFORMS IN MARCH 2020 DUE TO THE COVID-19 PANDEMIC. IN THE COMING YEAR, WELLNESS HOUSE WILL BEGIN TO RESUME IN PERSON SERVICES WHEN POSSIBLE AND APPROPRIATE.
