The Form 990 was reviewed by the Chief Financial Officer prior to filing. A copy of the Form 990 was sent to the governing body for their review prior to filing.

Form 990, Part VI, Section B, line 12C

Board members must annually complete a written conflict of interest questionnaire. Those individuals identified as having an actual, potential or perceived conflict of interest shall disclose all relevant information as requested, but shall not participate in any board discussions or vote taken with respect to their conflict of interest. The personnel policy manual of the Agency contains a written conflict of interest policy which all employees are required to uphold.

Form 990, Part VI, Section B, line 15

The following process for determining the compensation of the organization's CEO and CFO is the process used by the related organization, Casa Central Social Services Corporation: The process is performed by independent members of the governing body only and includes contemporaneous substantiation of the deliberation and decision. A comparability data study from the Alliance for Children and Families is presented to Executive Committee in determing compensation. The CEO's compensation is reviewed and approved by the Board. The CFO's salary is established by the President & CEO based upon similar responsibilities and other nonprofit comparisons.
