Governance Explanations

Form 990, Part VI, Section B, Line 12C

The by-laws of the organization contain a conflict of interest policy Article VI which was written to ensure that the members of the board of directors, personnel and/or consultants do not have conflicts of interest and do not use their relationship with the agency for personal gain. Compliance and enforcement is ongoing and conducted by the executive committee of the board and the executive director annually. Board members and employees are expected to advise the executive committee or executive director or supervisor of all outside business activities to prevent misunderstandings. Training is conducted annually at a board meeting and at an all-staff/volunteer meeting.

Form 990, Part VI, Section B, Line 15

The board of directors evaluates the performance and provides the executive director with an employment contract, which is reviewed annually and must pass by a simple majority vote.

Copies of the governing documents, conflict of interest policy and financial statements along with the 990 and application for tax exemption are provided upon request during normal business hours at the organizations location, as well as, posted on the Guidestar and the Attorney Generals website.

Program Service Expenses 1,241,458, Grants and allocations 0, Revenue 0 Subsidized Apartment Program SAP This one-year program supports shelter clients as they transition from communal living into independent housing. Residents live in agency-owned apartments while continuing to receive case management. During this time, they focus on paying down debts, repairing credit, and preparing to qualify for their own rental lease. Rapid Re-housing helps individuals and families move quickly from homelessness into permanent housing. Once accepted, clients have 30 days to secure a unit, with Home of the Sparrow covering the first months rent, security deposit and ongoing rental assistance for the duration of the grant up to 12 months. Clients also receive 12 months of case management and community connections to help them stay housed.

A copy of the 990 is sent electronically to the finance committee of the board of directors including the board treasurer for review. The recommendations of the finance committeee to accept/deny/change the 990 are given to the entire board at a bi-monthly meeting. The finance committee will approve the 990 for filing and the Board will see copy before filing.

The Sparrows Nest stores are located in Cary, McHenry, Mundelein, Palatine, Third Lake and Woodstock. One store does business under the name of Community Threads and is located in Arlington Heights. Inventory for all seven stores is received from donations and sellable inventory is resold in the store and non-sellable inventory is given to recycling companies. Income generated from store sales and recycling provides support for other programs within Home of the Sparrow. The thrift stores are also a zero-cost provider of clothing, furniture and housewares to Home of the Sparrow clients and clients of other partnering agencies in Northern Illinois.
