Complete copies of the U.S. Return of Organization Exempt From Income Tax (U.S. Form 990) and the Illinois Charitable Organization Annual Report (Illinois Form AG990-IL) were provided and reviewed by the board members prior to filing with the Internal Revenue Service and the Illinois Office of Attorney General.
