“All officers, directors and employees are required to report potential conflicts of interest to the Compliance Officer when his/her circumstances could create a conflict of interest, or prior to their arising, so that the health system can proactively review the report to identify actual and potential Conflicts of Interest. In addition, on an annual basis, members of the Board of Directors, Corporate Officers, and other key employees will be provided with a Conflict of Interest Questionnaire, which is used for purposes of reporting potential Conflicts of Interest. Central to this mission is a commitment to providing clinical programs and services that meet community health needs, while also pursuing continuous improvement to identify and understand future needs.”
“Endeavor Health Swedish Hospital, which opened in 1886, serves the culturally-diverse residents of Chicago's north and northwest side communities as a safety-net hospital, with a full-service hospital campus located in the Lincoln Square neighborhood. Swedish Hospital offers advanced clinical care in more than 50 medical specialties, which includes the Jack and Mayora Rosenberg Women's Health Center, offering 3-D mammography, and their Joint Commission-certified Primary Stroke Center. Swedish provides a full range of comprehensive health and wellness services offering 150+ wellness programs annually, and Chicago's only certified medical fitness center, Galter Life Center, and is one of six regional leads for Healthy Chicago Equity Zones. During 2024, Swedish Hospital total admissions were 10,841 and total inpatient days were 39,715. Physical Medicine/Rehab Services - Swedish Hospital is dedicated to providing a comprehensive Rehab Unit that combines the disciplines of physical therapy, speech therapy, occupational therapy and social work to help patients regain their strength and skills. The Rehab unit features a special room that simulates a home environment so patients can practice with home-like carpeting, furniture and fixtures while supervised by nurses. During 2024, 189,462 inpatient and outpatient treatments for rehab, speech and occupational therapy were provided for a total cost of $7,017,291. Cardiac Care and Rehab - The Cardiac Rehabilitation program offered by Swedish Hospital has improved the lives of many individuals with such innovative programs as "coronary Heart Disease Reversal." Swedish Hospital also offers a state-of-the-art Cardiac Catheterization Lab, and other diagnostic and preventive programs. During 2024, Swedish Hospital performed 2,788 procedures in the Cardiac Catheterization Lab for a total cost of $13,174,876. In addition to the program services listed above, Swedish Hospital also offers health screenings, health promotions, education, and prevention services. Swedish Hospital offers year-round health screenings to members of the community. Blood pressure screening events are provided in collaboration with the hospital's Stroke Education Coordinator. Each year free or reduced-cost cancer screenings are offered to the community. These included prostate, skin, cervical and breast screenings. Additionally, dozens of programs led by physicians, nurses, dietitians, personal trainers and other clinicians and experts from the hospital address topics of cancer prevention, treatment, nutrition, fitness and other healthy lifestyle recommendations. During 2024, Swedish Hospital hosted 79 programs at the Community Wellness Center on the hospital campus, with a total of 148 sessions. Additionally, Swedish Hospital participated in dozens of community outreach events in collaboration with community partners to serve thousands of local community members. Programs included health fairs, health education seminars, screenings and other health related activities. Community Wellness and Prevention programs continue to serve the community with more than 100 events held at the Hospital or in the community through collaboration with community partners. These events include health fairs, screenings, daytime and evening lectures, seminars, classes, Speakers' Bureau presentations, children's events and other health related activities. Throughout the year the Hospital collaborated with dozens of community organizations to provide wellness and prevention programs. Medical Education Swedish Hospital is a teaching hospital and part of Endeavor Health, with residency programs in emergency medicine, family medicine, internal medicine, and podiatric medicine and surgery. Swedish Hospital also has fellowship programs in critical care medicine and pulmonary critical care medicine. “The affiliates and subsidiaries of the System are primarily tax-exempt organizations under Internal Revenue Code Section 501(c)(3), and their related income is exempt from federal income tax. Accordingly, there is no material provision for income tax for these entities. Some of the income received by exempt entities is subject to taxation as unrelated business income. The System files federal income tax returns and returns for various states in the U.S. ASC 740-10, Income Taxes, requires that realization of an uncertain income tax position is more likely than not (i.e., greater than 50% likelihood of receiving a benefit) before it can be recognized in the consolidated financial statements. Furthermore, this interpretation prescribes the benefit to be recorded in the consolidated financial statements as the amount most likely to be realized assuming a review by tax authorities having all relevant information and applying current conventions. This interpretation also clarifies the financial statement classification of tax-related penalties and interest and sets forth new disclosures regarding unrecognized tax benefits. No amount was recorded for the year ended December 31, 2024. At year-end December 31, 2024, the System has a net operating loss carryforward of $416,494, which generated deferred tax assets of $121,678.
