Since we have dissolved and there is no longer a formal board or any meetings of such, the federal form 990 is presented to the treasurer by the independent cpa after it has been prepared. The treasurer then reviews the form 990 and compares it to the year-end financial statements. After reviewing the return and resloving any questions, it is then approved for signature and electronic filing.

Disclosure is discussed annually at the january board meeting and with new board members as they assume their board position. If necessary, conflict of interest forms are filled-out and signed by board members. Discussion and monitoring of these situations is done at board meetings as appropriate, as well as during executive committee sessions.

The executive committee performs an annual performance review with the executive director, the only contracted staff member, with input from board members as necessary or requested. There is no off-board review, but the process follows a formal, written format that is signed by all parties involved in the process once it is completed.

The endowment funds are held and administered entirely by the local community foundation, the putnam county community foundation, which is a totally unrelated organization. The intended use of the funds is to provide cash flow for our local united way in order for us to distribute allocated funds to our supported local organizations as voted on by our board of directors. This endowment fund was transferred along with the rest of our assets at the end of april 2021 to the united way of central indiana, inc. As part of our merger, and this is reflected on page 2 of schedule d in part v, line 1(e) in column a, current year.

THIS ENDOWMENT FUND WAS TRANSFERRED ALONG WITH THE REST OF OUR ASSETS AT THE END OF APRIL 2021 TO THE UNITED WAY OF CENTRAL INDIANA, INC. THE FIRST REPORT IS A COMMUNITY IMPACT REPORT TO BASICALLY PROVIDE INFORMATION ON THEIR PROJECT AND PROVIDE OUR BOARD WITH AN IDEA OF HOW THEY WILL POSITIVELY IMPACT THE QUALITY OF LIFE FOR THE BENEFICIARIES OF THE GRANT PROPOSAL. NEXT A FORMAL GRANT APPLICATION IS FILED WITH THE BOARD. ALL OF THESE DOCUMENTS ARE REVIEWED BY THE ALLOCATIONS COMMITTEE AND, IF THEY ARE AWARDED A GRANT, THEN A REPORT REVIEWING THE PROJECT IS DUE BY THE END OF THE FISCAL YEAR FOR WHICH THEY WERE AWARDED THE GRANT. DURING THE YEAR THE EXECUTIVE DIRECTOR OF THE BOARD MEETS WITH EACH GRANT RECIPIENT AT LEAST THREE TIMES TO REVIEW THE PROJECT AND THE DETAILS ON HOW THE FUNDS ARE BEING USED, AS WELL AS TO ASSESS IF THERE ARE ANY PROBLEMS OR CHALLENGES THAT NEED TO BE ADDRESSED.
