Mission

Through fundraising, Methodist Health Foundation champions the mission of Indiana University Health Adult Academic Health Center - improving the health of patients and community through innovation and excellence in care, education, research and service. The hospital has a rich heritage of delivering critical care - advanced, specialized and highly effective treatments - to critically ill and injured patients.

Methodist Health Foundation provides necessary funding for Indiana University Health Adult Academic Health Center.

Governance Explanations

Form 990, Part VI, Line 1A Delegate broad authority to A committee

The organization's executive committee shall consist of no less than 5 and no more than 9 members of the governing body. The executive committee shall have and may exercise all the powers of the board of directors.

Form 990, Part VI, Line 6 Classes of members or stockholders

Indiana University Health, a tax-exempt charitable organization, is the sole member of Methodist Health Foundation. IU Health approves the appointment of all members of the board of directors and approves significant decisions of the board.

Form 990, Part VI, Line 7A Members or stockholders electing members of governing body

Indiana University Health, a tax-exempt charitable organization, is the sole member of Methodist Health Foundation. Indiana University Health can designate an officer, director, or other person to vote or otherwise act on its behalf as the sole member. When vacancies occur on the board of directors, the nominating committee of the foundation shall nominate candidates and the member of the Foundation elects them into office.

Form 990, Part VI, Line 7B Decisions requiring approval by members or stockholders

Indiana University Health must approve the following decisions: 1. The articles of incorporation may be altered, amended or repealed only upon the affirmative vote of the member. 2. If the Foundation is dissolved, its assets must be transferred or conveyed for charitable health care related purposes to one or more organizations described in Section 501(c)(3) of the code selected by the member.

Form 990, Part VI, Line 11B Review of form 990 by governing body

The Finance Committee reviews a draft of the the Form 990 and addresses any questions with management and the independent tax preparers. A final copy of the form 990 is emailed to every voting member of the governing body before it is filed with the IRS.

Form 990, Part VI, Line 12C Conflict of interest policy

Every interested person is required to annually complete a conflict of interest survey. Potential conflicts are reviewed and investigated by the executive committee, which is made up of board members. The executive committee is charged with enforcing compliance with the conflict of interest policy at board meetings and ensures that an individual board member with a potential conflict recuse themselves from the vote.

The Human Resource Committee, which consists of board members, obtained a custom compensation survey from an independent agency in October 2014. The intended use of the endowment funds is to support the research, education programs and equipment of the Indiana University Academic Health Center which will enable it to continue providing innovative and best practice procedures to patients.

Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote

The Internal Revenue Service has determined that the Foundation is a tax-exempt organization as defined in Section 501(c)(3) of the Internal Revenue Code and is exempt from federal income taxes on related income. The Foundation is not considered to be a private foundation. Twentyfive has elected to be treated as an association taxable as a corporation. Current accounting standards require the Foundation to disclose the amount of potential benefit or obligation to be realized as a result of an examination performed by a taxing authority. For the years ended December 31, 2015 and 2014, management has determined that the Foundation does not have any tax positions that result in any uncertainties regarding the possible impact on the Foundation's financial statements. The Foundation is no longer subject to examination by taxing authorities for years before 2011. The Foundation does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. The Foundation recognizes interest and/or penalties related to income tax matters in income tax expense. The Foundation did not have any amounts accrued for interest and penalties at December 31, 2015 and 2014.

IRS990/Desc0This grant supported the acquisition of a True Beam STx linear accelerator. Linear accelerators are used to produce high energy electrons and X-rays to deliver radiation treatments. The True Beam STx has the capability to both treat standard radiation plans and deliver specialized treatment plans not possible with standard linear accelerators (stereotactic radiosurgery and stereotactic body radiation therapy). Treatments delivered with the True Beam STx will benefit patients with cancer diagnoses by providing stereotactic capabilities currently delivered by other machines that are at the end of their functional lives. Upgrading capabilities with this machine will maintain the ability to deliver these specialized radiation treatments and provide them more efficiently. This will reduce the amount of time patients are under treatment and makes the experience more pleasant for them. The True Beam STx will support the care provided at IUH Methodist Hospital, IUH University Hospital, and IUH Simon Cancer Center by multiple disciplines in the Cancer and Neuroscience service lines. Treatments on this device will also benefit patients from other cities in Indiana to receive specialty care at the Academic Health Center.
