Form 990, Part III, line 28:Crescent Cares Community Health Worker Program- To address Sickle Cell Disease (SCD) disparities in Philadelphia and the Delaware Valley , Crescent Foundation established a Community Health Worker program, passionate advocates for individuals living with SCD, to deliver the highest level of compassionate healthcare and social services. In addition to providing support to patients and their families, they conduct research and outreach, offer health education and case management. There were a total of 165 Crescent Cares referrals, of which 77 were for the Equitable Access To Rejuvenating Therapeutic Healthcare (E.A.R.T.H.) program which provides access to alternative therapeutic healthcare such as doula services, midwifery, therapy, acupuncture, and massage therapy. In addition, we received 265 transportation requests.

Form 990, Part III, line 28: ARPA Lead Abatement Program - At Crescent Foundation, the American Rescue Plan Act (ARPA) Housing Program worked to help qualified homeowners, landlords, and renters improve the safety of their properties and homes by testing for and remediating lead paint for any child with a positive lead toxicity test. Seven constituents were supported by the ARPA program and Crescent Foundation received two additional requests for lead abatement support.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt0Form 990, Part III, line 28:Crescent Cares Community Health Worker Program- To address Sickle Cell Disease (SCD) disparities in Philadelphia and the Delaware Valley , Crescent Foundation established a Community Health Worker program, passionate advocates for individuals living with SCD, to deliver the highest level of compassionate healthcare and social services. In addition to providing support to patients and their families, they conduct research and outreach, offer health education and case management. There were a total of 165 Crescent Cares referrals, of which 77 were for the Equitable Access To Rejuvenating Therapeutic Healthcare (E.A.R.T.H.) program which provides access to alternative therapeutic healthcare such as doula services, midwifery, therapy, acupuncture, and massage therapy. In addition, we received 265 transportation requests.

Seven constituents were supported by the ARPA program and Crescent Foundation received two additional requests for lead abatement support.
