A copy of the form 990 was provided to all board members for review prior to filing the return.

Financial Statement Notes

Pt X, Line 2

Healthvisions midwest is exempt from income taxes under section 501(c)(3) of the united states internal revenue code and qualifies for the 60 percent charitable deduction limitation. Healthvisions midwest has been classified as an organization that is not a private foundation under section 509(a) of the internal revenue code. Healthvisions midwest is also exempt from state income taxes. Healthvisions midwest is subject to federal income tax on any unrelated business income. Healthvisions midwest provides liabilities for uncertain income tax positions when a liability is probable and estimable. Management believes that it has appropriate support for any tax positions taken or expected to be taken and as such, does not have any uncertain tax positions that should be recognized, measured, or disclosed in the financial statements. Management believes healthvisions midwest is no longer subject to examination by taxing authorities for years before december 31, 2020.

VOLUNTEERS RECEIVE TRAINING AND COMPREHENSIVE SCREENING, INCLUDING CRIMINAL BACKGROUND CHECKS. INFORMATION AND ASSISTANCE (I&A) ASSISTED SENIORS WITH ACCESSING SERVICES AND RESOURCES THAT ENABLE THEM TO MAINTAIN THEIR INDEPENDENCE. SENIORS DEVELOP A CLOSE CIRCLE OF FRIENDS AS THEY SHARE EXPERIENCES TOGETHER. TELEPHONE REASSURANCE CALLS (TRC) ENABLE SENIORS TO REMAIN INDEPENDENT AND LIVE IN THEIR HOMES. IRS990ScheduleA/Form990ScheduleAPartVIGrp/ExplanationTxt0HealthVisions Midwest relied heavily in 2021, 2022 and 2023 on funding from our founding sponsor, the Poor Handmaids of Jesus Christ, and a grant from a related agency, St. Joseph Community Health Foundation. Due to these funds, the organization fell below the 33 1/3% revenue requirement from public and government sources. HealthVisions has now taken steps to increase revenue from public and government sources. We are also transitioning to receive less support from the sponsor and replace those funds by developing a robust list of donors. In 2023, HealthVisions established inaugural fundraiser events to build donors and aid in replacing the funds received from the Sponsor. The fundraisers were profitable and motivated potential donors. The organization now works with grants consultants to increase funding opportunities from government agencies and foundations whose funding priorities align with the HealthVisions Midwest mission. Finally, HealthVisoins executives are working with fundraising and development professionals to target and communicate with potential donors. The goal is to build strong relationships in local markets that yield revenue.
