INFORMATION AND ASSISTANCE (I&A) ASSISTED SENIORS WITH ACCESSING SERVICES AND RESOURCES THAT ENABLE THEM TO MAINTAIN THEIR INDEPENDENCE. I&A PROVIDED CONTACTS TO SENIORS TO MAKE HEALTHIER LIFESTYLE CHOICES AND ACTED AS ADVOCATES FOR MEDICARE, MEDICAID, BENEFIT ACCESS, LOW INCOME HOME ENERGY ASSISTANCE PROGRAM, ETC. TO IMPROVE THEIR QUALITY OF LIFE AND REDUCE HEALTH DISPARITIES. CIRCLE OF FRIENDS (COF) REDUCES SOCIAL ISOLATION AND LONELINESS FOR SENIORS BY PROVIDING INTERACTIVE GROUP ACTIVITIES, HEALTH EDUCATION, AND OUTINGS. SENIORS DEVELOP A CLOSE CIRCLE OF FRIENDS AS THEY SHARE EXPERIENCES TOGETHER. TELEPHONE REASSURANCE CALLS (TRC) ENABLE SENIORS TO REMAIN INDEPENDENT AND LIVE IN THEIR HOMES. THE CALLS HELP SENIORS IMPROVE THEIR QUALITY OF LIFE BY CONNECTING THEM TO VOLUNTEERS WHO CARE ABOUT THEIR WELL-BEING. | IRS990ScheduleA/Form990ScheduleAPartVIGrp/ExplanationTxt | 0 | HealthVisions Midwest relied heavily in 2021, 2022 and 2023 on funding from our founding sponsor, the Poor Handmaids of Jesus Christ, and a grant from a related agency, St. Joseph Community Health Foundation. Due to these funds, the organization fell below the 33 1/3% revenue requirement from public and government sources. HealthVisions has now taken steps to increase revenue from public and government sources. We are also transitioning to receive less support from the sponsor and replace those funds by developing a robust list of donors. In 2023, HealthVisions established inaugural fundraiser events to build donors and aid in replacing the funds received from the Sponsor. The fundraisers were profitable and motivated potential donors. The organization now works with grants consultants to increase funding opportunities from government agencies and foundations whose funding priorities align with the HealthVisions Midwest mission. Finally, HealthVisoins executives are working with fundraising and development professionals to target and communicate with potential donors. The goal is to build strong relationships in local markets that yield revenue. |
