Mission

Future Choices, Inc. was organized to provide access to services and to conduct activities to ensure that people with special needs and their families are empowered to make personal choices from a wide range of acceptable options which lead to the greatest level of independence.

The mission is to provide unlimited options for minorities, youth, and Hoosiers with disabilities. The Organization is an open door, call in service provider that values the diginity of every person regardless of race, color, religion, sex, family status, national origin and disabilities.

Governance Explanations

Form 990, Part VI, Section B, Line 11A, 11B

The finance committee of the board and the executive director review the Form 990. All voting members have access to the Form 990 before it is filed. After reviewing the tax return, the board members recommend and approve the return for filing. The approval is documented in the board minutes.

Form 990, Part VI, Section B, Line 12 A, B, C

The Board members review conflict of interest policy annually and document the review in the minutes. The staff and volunteers are constantly made aware of the conflict of interest policies during staff and board member meetings. Members and employees are made aware of the negative impact issues of such nature could have on the goals and objectives of the organization.

Form 990, Part VI, Section B, Line 15A, 15B

There are 9 Centers in the State of Indiana. The National Council for Independent Living Centers have a scale for each State to determine the pay scale for the Executive Directors. The Board members will review the National information according to education, experience and then review and approve the salary for the Executive Director.

Form 990, Part VI, Section C, Line 19

The organization undergoes an annual financial audit conducted by an independent CPA firm. The financial audit with the other documents are made available to the general public upon request. The audit documents are filed with the Indiana State Board of Accounts as part of the compliance procedures.
