Activity 2

Tangram operates supervised group living sites in greenfield, in and indianapolis, in. Each site accommodates 6 individuals with developmental disabilities living together in the community. There is 24-hour supervision by paid staff that provides assistance and training to help individuals develop daily living skills with an emphasis on skills that foster self sufficiency and community participation. In the year ended june 30, 2011, 18 individuals were served by this program.

Activity 3

Tangram provides behavioral consultation and training. Behavioral consultation is a service that begins with a thorough mental health assessment, utilizing a functional behavioral assessment (fba) approach. Subsequently, a behavioral support plan is developed. The consultant then trains the individual and team to implement the plan and collects objective data to measure its effectiveness. The consultant remains an important component of the client team and provides expertise and guidance pertaining to all aspects of behavioral performance and mental health. Tangram provides behavioral training to parents, schools, organizations and others, as requested. Tangram's behavioral training includes: assessments and behavioral support plans, behavioral support data collection and analysis, positive behavioral supports, inclusive schools and other settings, psychotropic medication, general behavioral support, mental illness and developmental delay, and harnessing the power of the support team. Tangram's behavioral health team provides counseling in both individual and small group settings. Counseling services are available in tangram's office or the home setting. The highly skilled therapists assess client needs and develop a subsequent treatment plan. In the year ended june 30, 2011, 95 individuals were served by this program.
