“The board of directors is presented with a copy of the form 990 and a summary of the return before it is filed. The board members are asked to review the form 990 and provide comments. Any questions or concerns are addressed before the return is filed.”

“Every member of the governing body and the officers are required to annually sign a conflict of interest statement and disclose any conflicts of interest. If any conflicts exist, the individual with the conflict will abstain from voting on matters related to the conflict of interest. If a conflict of interest develops during the year, a written statement outlining the details is required to be submitted to the board. The executive director monitors and enforces this.”

“During 2012, the organization hired an independent accounting firm to perform a comprehensive compensation review including an analysis of job descriptions, comparability data, and recommendations for pay rates. This data was incorporated into our compensation system. The Policy Committee reviewed the recommendations and approved the pay ranges and adjustments. The Board approved the changes to the compensation system based on the review and the recommendation of the Policy Committee. The review studied the salaries of the top management official and top financial official. The decisions are documented. This process was last undertaken in 2012.”

“The board of directors established an endowment fund in 2002 to celebrate the 20th anniversary of the organization. The purpose of the fund is to enhance lifedesign's mission to partner with people with disabilities and our communities to bring about self-directed and enriched lives through information, education, and support.”

Part X, Line 2:

“Accounting principles generally accepted in the United States of America require management to evaluate tax positions taken by LIFEDesigns and recognize a tax liability if LIFEDesigns has taken an uncertain position that more likely than not would not be sustained upon examination by various federal and state taxing authorities. Management has analyzed the tax positions taken by LIFEDesigns, and has concluded that as of December 31, 2015 and 2014, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the accompanying consolidated financial statements. LIFEDesigns is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress. LIFEDesigns filed its federal and state income tax returns for periods through December 31, 2014. These income tax returns are generally open to examination by the relevant taxing authorities for a period of three years from the later of the date the return was filed or its due date (including approved extensions).”
