Governance Explanations

Form 990, Part VI, Section B, Line 11B

The executive director provides a copy of the form 990 at the regularly scheduled monthly board meeting for all members to review. It is presented during the meeting along with the audited financial statements.

Form 990, Part VI, Section B, Line 12C

To monitor and enforce compliance with the conflict of interst policy every year, the board is required to review and sign an affadavit that they are in compliance with policy. Employees at the agency are also required to review and sign yearly as well.

Form 990, Part VI, Section B, Line 15A

O determine pay rate increases the executive board meets and reviews the data and budget of the agency along with the other comparable jobs in the industry. From this data they determine the appropriate pay increases for all employees of the agency. The increases are then voted on by the board and documented in the board minutes.
