“Fisher-titus medical center (ftmc) is an independent non-profit community healthcare organization governed by a board of directors composed of area residents including business and community leaders, physicians, and other interested individuals. Ftmc is composed of the following five operating divisions: fisher-titus memorial hospital the hospital provides inpatient and outpatient healthcare services to the residents of huron, erie and the adjacent counties of sandusky, crawford, lorain, ashland, and richland. In 2024, the hospital provided 12,891 days of inpatient care to 3,421 patients. The hospital had 135,939 outpatient and 27,697 emergency room visits during the year. The norwalk memorial home the norwalk memorial home provides skilled and long-term care in its transitional and extended care units. During 2024, the home served 394 residents and provided 22,414 days of care. The home health center home health agency the home health center provided 12,310 home visits to 237 different patients in 2024. The carriage house of fisher-titus opened in 1998, the carriage house is a 48-unit assisted living facility, which provided 10,347 days of assisted living care to 47 residents in 2024. Facility and program expansions in february 2024, fisher-titus welcomed mohamad mouchli, md, gastroenterologist, to its team of providers. Dr. Mouchli specializes in caring for patients with disorders and diseases that affect the digestive system. He joins fisher-titus with gastroenterology experience from the cleveland clinic. His passion for gastroenterology shows in his membership in the american medical association and the american college of gastroenterologists. He has also contributed to writing nearly 50 peer-reviewed articles, along with appearing in the media and news coverage to educate others on important gastroenterology issues. In march 2024, fisher-titus celebrated the official opening of the willard health center in willard, ohio. As we expand in the willard community, the new health center now includes family medicine, cardiology care, cardiovascular testing, physical therapy, laboratory services, behavioral health services in a centralized and convenient location. In august, two pediatrics providers were added to the new willard location expanding services along with new x-ray services. In april, health screenings were offered for early detection of colorectal cancer and for general wellness testing. Our dietetics team offered grocery shopping classes at our local supermarket to provide tips on how to spot nutritional "traps" in prepared foods that can have negative health impacts over time. Lastly, our healthy kids, healthy you program kicked off in several local school systems focused on young learners and encouraging healthy eating and portion control. In september 2024, fisher-titus broke ground on the wakeman health center, an expansion to the existing wakeman family medicine building. This expansion will bring new services to the wakeman community, including pediatrics, radiology, urology, and rehab services, and expand access to family medicine and podiatry. This health center concept will ensure that area residents have convenient access to the care they need for years to come. In december, fisher-titus opened a new 3800-square-foot office specializing in urology and general surgery in bellevue, ohio. The new setting allows our providers to perform a wider range of medical and diagnostic procedures in-office, at a lower cost of care, while saving patients the hassle of scheduling a separate trip to a hospital or other facility and making for much faster diagnosis and treatment. The urology team at this new location consists of patrick waters, md, kathy lue, md, jennifer perry, pa-c, aurora orzech, fnp-c, and alysha galea, fnp-c. “August 2024 -fisher-titus partnered with the local alzheimer's association to begin hosting monthly caregiver support groups onsite at the hospital. These support groups are conducted by trained facilitators and are a safe and supportive place for individuals to care for a loved one with dementia. -the western reserve field and track complex officially opened in august as the 2024 school year began. Fisher -titus is proud to have been a key sponsor of this project providing for a health and wellness community asset. September 2024 -the 2024 healthy heart walk held on september 7th to encourage healthy heart behavior and drive community involvement. Representatives from the fisher-titus heart & vascular department offered education on two essential steps to save a life in less than ninety seconds. October 2024 -the fisher titus occupational health team visited new london schools to participate in their employee health fair assisting with their employee wellness program. These wellness events provided screenings that ranged from body mass index (bmi), lab testing, blood pressure checks, healthy living education, and vaccinations. November 2024 -wellness prescribed podcast "what to expect in the er" featuring john parente, md. As director of the fisher-titus emergency department, he tells what they commonly see in the emergency room, what you can expect when you visit the er, and prevention tips to avoid visiting the er. December 2024 -fisher-titus sponsored multiple holiday events across huron county to drive community involvement and support local communities. These included: norwalk's light up norwalk who london in new london hometown holidays in plymouth willard christmas parade education and research - to ensure that we have the quality workforce we need to care for our community in the future, we are committed to the education of current and future caregivers, and we collaborate with local high schools and ohio universities to provide internships, instructors, classroom space, and clinical training. We are pleased to be able to offer comprehensive learning experience utilizing our state-of-the-art facilities and advanced technology. The healthcareers program conceived post-pandemic continued into its full second year. This program continued successfully to identify and recruit local high school students who are interested in future healthcare careers. The lpn to rn clinical ladder program continues to support the advancement and retention of our clinical nurses providing care to the huron county community and beyond. In 2024, fisher-titus medical center, in partnership with erie huron ottawa vocational education (ehove) career center, magruder hospital, and terra state community college, is proud to announce it has been awarded a $4 million grant from the health resources and services administration (hrsa), an agency of the u.s department of health and human services. This funding aims to improve the supply and geographic distribution of the registered nurse (rn) workforce throughout erie, ottawa, sandusky, seneca, and huron counties. Over the four-year period, funding will be utilized to: 1) recruit and train 250 licensed practical nurses (lpns) to become rns through lpn to rn bridge programs at ehove and terra state community college. 2) provide direct support to students to break down barriers that licensed practical nurses (lpns) may experience that prevent them from advancing their careers, such as transportation, childcare, and technology costs. 3) increase hands-on clinical experiences in both healthcare and community-based settings, providing students with longitudinal experiential training and additional preceptorship opportunities. While lpn to rn programs currently exist in the region, faculty shortages and lack of preceptors have made it difficult to provide the needed level of supervision and training students require to be successful in the field. News and world report's list of best nursing homes community health and outreach programs: fisher-titus medical center's community benefits extend outside our walls to families, business, schools, churches and neighborhoods through outreach programs and other coordinated care activities that make our communities healthier places to live, work and worship. Our goal is to help all residents equally access the care they need and help them learn to manage their conditions and live healthier, safer lives. In 2023, a community health needs assessment was completed and published, and conducted, in partnership with huron county public health department, every three years. Fisher-titus was a leader in the process and partnered with health-related and civic organizations throughout the county to both fund the project and assist in the assessment process. From that report, the board of directors of fisher-titus medical center determined that certain health needs identified in the assessment should be addressed as the highest. The priorities include supporting initiatives geared to healthy weight; improving access to mental health services; reducing substance abused through prevention, screening, and treatment; improving access to care; and improving the public health infrastructure. Fisher-titus supports other area non-profit community organizations that are committed to community health improvement through financial investments, in-kind service, and employee volunteerism. Many of our community benefit programs are organized to work collaboratively with community groups, schools, and government organizations to effectively leverage resources and address identified community needs. In 2023, we offered community health education programs, including school-based programs, support groups, community-based health fairs and screenings, and walking and fitness programs that impacted our residents through 144,428 encounters.
