Geography & ReachstART the Talk will be permanently installed across all 20 Cincinnati Health Department CHD primary-care clinics including 13 school-based health centers. These sites are strategically located in Hamilton Countys highest-need neighborhoods including Avondale Price Hill and Walnut Hills designated as Medically Underserved Areas MUA P and Health Professional Shortage Areas HPSA.Target Population & Demographics: The program will serve an intergenerational population from infants and students to adult caregivers. In 2022 these sites recorded 118,365 visits from 44,692 unique individuals.Socio-Economic Status: 83% of patients live at or below the federal poverty level facing concentrated housing instability and food insecurity.Health Burden: The community experiences elevated rates of asthma obesity and diabetes.Projected Impact: We anticipate over 100,000 annual interactions with our multilingual English Spanish French Chinese QR-integrated resources reaching 25,000+ youth and their families.
