Form 990, Part VI, Section B, Line 11

Process the organization uses to review the form 990: the board of directors is responsible for performing a review of the form 990 prior to filing. Upon completion of the tax return draft by the accounting firm, the finance director performs a review of the tax return and presents it to the senior associate executive director for approval. Then form 990 is forwarded to the board of directors. A meeting is held and the form 990 is presented to the board by the senior associate executive director and finance director. Upon completion of the presentation and the discussion related to any questions, concerns, issues or clarification, the board will be required to vote on the approval for the filing of the tax returns. Once the vote is approved by the board form 990 can be filed.

Form 990, Part VI, Section B, Line 12C

Process the organization uses to monitor and enforce compliance with the conflict of interest policy: any conflict of interest, potential conflict of interest or appearance of a conflict of interest should be immediately reported in writing by an employee with knowledge of the conflict to the director of human resources and verbally to his/her immediate supervisor. The director of human resources along with the senior associate executive director will determine an appropriate response to the report. Steps to be followed to making such a determination include: 1. A complete and thorough investigation of the actual or potential conflict of interest. 2. Elimination of any prohibited conflict of interest and alleviation of any harm from the conflict of interest to the agency, its clients, and others whenever practical. 3. Assessment and imposition of appropriate guidance process action. 4. Depending on the severity of the conflict of interest, the human resources director and the senior associate executive director may consult with the executive director regarding action(s) to be taken. 5. Annual review of employee handbook and conflict of interest policy.
