Form 990, Part XII, Line 2: Change of Oversight or Selection Process

The Disability Foundation is included in the audited consolidated financial statements of The Dayton Foundation. The Dayton Foundation has a committee that assumes responsibility for oversight of the audit of the financial statements and the selection of an independent accountant.

Relationship to Dayton Foundation and payroll services

The Disability Foundation is a Type I Supporting Organzation under the auspices of the Dayton Foundation. In additon to overseeing the daily activites and monitoring the finances of the Disability Foundation, the Dayton Foundation also provides payroll services to the Disability Foundation. The Dayton Foundation pays Disability Foundation staff members and then is reimbursed, for those staff costs, from the Operating Budget of the Disability Foundation.

Part X : FIN48 Footnote

The Disability Foundation is included in the audited consolidated financial statements of The Dayton Foundation. The following footnote is included with the consolidated financial statements:The Foundation determines the recognition of uncertain tax positions, if applicable, that may subject the organization to unrelated business income tax by applying a more-likely-than-not recognition threshold and determines the measurement of uncertain tax positions considering the amounts and probabilities of the outcomes that could be realized upon ultimate settlement with tax authorities. Based on its review, management does not belive the Foundation has taken any material uncertain tax positions, including any position that would place the Foundation's exempt status in jeopardy as of June 30, 2018.

Durin FY 2018:* We now have over 800 trust accounts across 36 counties in Ohio* Established 100 new accounts* Distributed over $2M to beneficiaries* For the first time in the foundation's history exceeded 800 participants* Have Assets over $21M* Refined systems of reconciliation for both the flexible spending and the annuity trusts* Internal accountability controls have bee refined and strengthened* Deposits and distribution requests are now electronically transmitted to the bank, eliminating many problems associated with mailing, reducing time & increasing security* developed system to accept electronic deposits* refined website to make more attractive and easy to navigate to useful information* an independent charitable gift annuity risk audit was completed in March by "Charitable Solutions, LLC" ( Bryan Clontz, Phd, CFP) and found our annuity trust to be exceptionally financially healthyWe are making a concerted effort to reconnect in person with beneficiaries, where possible. We take our fiduciary responsibility to our beneficiaries very seriously. It is our responsibility to work with the beneficiary and their personal representative to manage the trust for their benefit. This requires planning and a good sense of the challenges presented to the beneficiary by the disability with which they live.It is important to gain visibility for our program, not only in the Miami Valley but around the state of Ohio. We frequently make presentations to professional advisors, county boards and community organizations.Of particular note during FY 2018, we also broadened exposure by:* exhibiting at the Ohio Providers Resources Association Fall Conference 2017 in Columbus Ohio;*being the sole sponsor and exhibitor at the Dayton Bar Association Probate Law Conference;* in March 2018, we not only had an exhibition table but also led off the OHio Bar Association Elder Law Conference in Columbus with a panel discussion joining the Community Fund Management Foundation of Cleveland. We presented "Pooled Trusts: Everything You wanted to know but were afraid to ask" to 180 attorney from across Ohio.
