Governance Explanations

Form 990, Part VI, Section B, Line 11B

The healthpath foundation of ohio posts the form 990 to their website prior to filing with the irs. Board members are contacted to inform them that it is available and are given time for review.

Form 990, Part VI, Section B, Line 12C

Board members are required to complete a conflict of interest questionnaire at the first board meeting each year. The president evaluates those for any conflicts. Since inception, we have not had a conflict of interest.

Form 990, Part VI, Section B, Line 15

Each fall an operating budget is developed that inclues recommended salary and benefits increases for officers. The budget is submitted to the board's governance committee, along with annual competitive salary information. The governance committee reviews the infformation and makes a recommendation to the full board to either accept the proposed executive compensation levels in the budget for the next year or wheater to modify it. The board decides wheither to accept the recommendation or make changes at its annual march board meeting. Annual changes to compensation go into effect on march 1st each year.

FOR EXAMPLE, HEALTHPATH BECAME THE FISCAL AGENT FOR THE CHILDREN'S ORAL HEALTH ACTION TEAM (COHAT) AND CHANGED THE NAME TO ORAL HEALTH OHIO, COMPLETED A STRATEGIC PLANNING PROCESS, HIRED A FULL-TIME COORDINATOR AND ATTRACTED OUTSIDE FUNDING TO SUPPORT THE WORK. HEALTHPATH IS ALSO THE LEAD IN CREATING A DENTAL ROTATION AND RESIDENCY PROGRAM IN SOUTHEASTERN OHIO, JOINTLY FUNDING THE PROJECT WITH THE APPALACHIAN REGIONAL COMMISSION
