“On universal health coverage in latin america. Medicc review's spanish edition was launched at two venues in april 2015: one in chile, by dr. Etienne and dr. Vega; and a second in cuba by medicc review editors and the paho representative at cubasalud 2015. These were available in print, thanks to a grant from paho. -publication of a special 2014 issue, chronic kidney disease hits agricultural communities, in response to a call by then minister of health of el salvador, dr. Maria isabel rodriguez, who brought this epidemic to the attention of the international community at paho and other forums. Mr was the first journal to dedicate an entire issue to this problem facing poor farmers in central america, sri lanka and other places where it has caused at least 20,000 deaths. Hypotheses include poor working conditions and pesticide exposure. Cuban paho consultants, the president of the international society of nephrology and authors from el salvador, nicaragua, the usa, sri lanka and mexico, contributed to the issue. An example of mr addressing an important regional/global health equity problem. -annual attendance at the council of science editors meetings, recently including presentation by issue coordinator lila castellanos. -organization of a second trip for us medical/population health journal editors, resulting in several positive editorials on the cuban health system, and continuing cooperation. -continuous updating of best editorial practices through regular seminars by dr. Lila castellanos for editorial staff. -presentation of the journal at institutions throughout cuba, to encourage manuscript submissions; as well as at international conferences in cuba, the usa and elsewhere. -organization of content-related forums involving authors and lively debate of the content of specific issues; and of events to honor editorial board members and most-read authors.”

“Md pipeline to community service: recognizing the challenges inherent in transitioning from medical training in cuba to residency and practice in the u.s., medicc initiated its md pipeline to community service program in 2007 to assist students and graduates in taking the final steps toward becoming physicians in underserved u.s. Communities. In the nine years since its inception, the md pipeline program has grown to include three major components that provide support from the first year of medical school through graduation and residency training: -mnisi scholarship: defrays the significant financial burden of preparing for and taking the three licensing exams required for md recognition in the united states -community connections: offers students internships and clinical rotations in public hospitals and community clinics around the country, providing them with critical hands-on experience with the u.s. Healthcare system and a chance to build professional networks within safety-net systems -pathways to residency: supports and guides u.s. Elam graduates through the residency application and matching process we are tremendously proud of the results this program has achieved overall thus far: -our students have the same or better pass rates for the three u.s. Licensing exams as other international medical graduates -in 2015, our students had an 83% match rate to u.s. Residency programs. In comparison, the overall match rate for u.s. Citizen graduates of international medical schools was only 53% -there are now 65 u.s. Elam graduates in residency training or practice nationwide. Of these, 90% are in primary care professions; 65% are serving in federally designated areas of need, and 100% are working with underserved communities. Specific metrics for the past year include: -mnisi scholarship: in 2015, 75% of step 1 exam takers who were supported by the mnisi scholarship passed the exam on the first try. 85% of step 2 ck takers passed and 75% of step 2 cs takers. Students and graduates with low test scores and/or failed exams continue to be a main concern for the mnisi scholarship program, and md pipeline program staff plan to address this issue into 2017 and beyond by increasing the number of meetings in cuba for exam support and creating action plans for students with failed exams. -community connections: goals for this program were met or exceeded in 2016. 26 rotations, including 7 clerkships and 19 observerships were completed throughout the united states. Once again, all students who participated ranked their experiences as highly formative and positive, and the vast majority of preceptors agreed, saying they would provide a letter of recommendation to students and host another student in the future. Looking forward, we plan to focus on assessing the programs impact on reinforcing student commitment to work with underserved populations. -pathways to residency: our pathways to residency program continues to grow and shift in response to and to better support the needs of match-ready elam graduates. The past two years, 90% or more of pathways participants have reported program components as integral to their match success. In 2016, there are 15 graduates in the match being supported by the pathways program. As 2017 continues to progress, we plan to expand this support to include a focus on re-integration for new graduates, and continue to offer resources and support based on evaluations and needs expressed by the students and graduates themselves. Communications: medicc's biggest policy win was the result of two solid years of policy and communications work. The october 2016 presidential directives authorized joint medical research between us and cuban centers, and also swept away the roadblocks to moving cuban-developed medicines through the normal fda regulatory process. IN THE NINE YEARS SINCE ITS INCEPTION, THE MD PIPELINE PROGRAM HAS GROWN TO INCLUDE THREE MAJOR COMPONENTS THAT PROVIDE SUPPORT FROM THE FIRST YEAR OF MEDICAL SCHOOL THROUGH GRADUATION AND RESIDENCY TRAINING: -MNISI SCHOLARSHIP: DEFRAYS THE SIGNIFICANT FINANCIAL BURDEN OF PREPARING FOR AND TAKING THE THREE LICENSING EXAMS REQUIRED FOR MD RECOGNITION IN THE UNITED STATES -COMMUNITY CONNECTIONS: OFFERS STUDENTS INTERNSHIPS AND CLINICAL ROTATIONS IN PUBLIC HOSPITALS AND COMMUNITY CLINICS AROUND THE COUNTRY, PROVIDING THEM WITH CRITICAL HANDS-ON EXPERIENCE WITH THE U.S. HEALTHCARE SYSTEM AND A CHANCE TO BUILD PROFESSIONAL NETWORKS WITHIN SAFETY-NET SYSTEMS -PATHWAYS TO RESIDENCY: SUPPORTS AND GUIDES U.S. ELAM GRADUATES THROUGH THE RESIDENCY APPLICATION AND MATCHING PROCESS WE ARE TREMENDOUSLY PROUD OF THE RESULTS THIS PROGRAM HAS ACHIEVED OVERALL THUS FAR: -OUR STUDENTS HAVE THE SAME OR BETTER PASS RATES FOR THE THREE U.S. LICENSING EXAMS AS OTHER INTERNATIONAL MEDICAL GRADUATES -IN 2015, OUR STUDENTS HAD AN 83% MATCH RATE TO U.S. RESIDENCY PROGRAMS. IN COMPARISON, THE OVERALL MATCH RATE FOR U.S. CITIZEN GRADUATES OF INTERNATIONAL MEDICAL SCHOOLS WAS ONLY 53% -THERE ARE NOW 65 U.S. ELAM GRADUATES IN RESIDENCY TRAINING OR PRACTICE NATIONWIDE. OF THESE, 90% ARE IN PRIMARY CARE PROFESSIONS; 65% ARE SERVING IN FEDERALLY DESIGNATED AREAS OF NEED, AND 100% ARE WORKING WITH UNDERSERVED COMMUNITIES. SPECIFIC METRICS FOR THE PAST YEAR INCLUDE: -MNISI SCHOLARSHIP: IN 2015, 75% OF STEP 1 EXAM TAKERS WHO WERE SUPPORTED BY THE MNISI SCHOLARSHIP PASSED THE EXAM ON THE FIRST TRY. 85% OF STEP 2 CK TAKERS PASSED AND 75% OF STEP 2 CS TAKERS. STUDENTS AND GRADUATES WITH LOW TEST SCORES AND/OR FAILED EXAMS CONTINUE TO BE A MAIN CONCERN FOR THE MNISI SCHOLARSHIP PROGRAM, AND MD PIPELINE PROGRAM STAFF PLAN TO ADDRESS THIS ISSUE INTO 2017 AND BEYOND BY INCREASING THE NUMBER OF MEETINGS IN CUBA FOR EXAM SUPPORT AND CREATING ACTION PLANS FOR STUDENTS WITH FAILED EXAMS. -COMMUNITY CONNECTIONS: GOALS FOR THIS PROGRAM WERE MET OR EXCEEDED IN 2016. 26 ROTATIONS, INCLUDING 7 CLERKSHIPS AND 19 OBSERVERSHIPS WERE COMPLETED THROUGHOUT THE UNITED STATES. ONCE AGAIN, ALL STUDENTS WHO PARTICIPATED RANKED THEIR EXPERIENCES AS HIGHLY FORMATIVE AND POSITIVE, AND THE VAST MAJORITY OF PRECEPTORS AGREED, SAYING THEY WOULD PROVIDE A LETTER OF RECOMMENDATION TO STUDENTS AND HOST ANOTHER STUDENT IN THE FUTURE. LOOKING FORWARD, WE PLAN TO FOCUS ON ASSESSING THE PROGRAMS IMPACT ON REINFORCING STUDENT COMMITMENT TO WORK WITH UNDERSERVED POPULATIONS. -PATHWAYS TO RESIDENCY: OUR PATHWAYS TO RESIDENCY PROGRAM CONTINUES TO GROW AND SHIFT IN RESPONSE TO AND TO BETTER SUPPORT THE NEEDS OF MATCH-READY ELAM GRADUATES. THE PAST TWO YEARS, 90% OR MORE OF PATHWAYS PARTICIPANTS HAVE REPORTED PROGRAM COMPONENTS AS INTEGRAL TO THEIR MATCH SUCCESS. IN 2016, THERE ARE 15 GRADUATES IN THE MATCH BEING SUPPORTED BY THE PATHWAYS PROGRAM. AS 2017 CONTINUES TO PROGRESS, WE PLAN TO EXPAND THIS SUPPORT TO INCLUDE A FOCUS ON RE-INTEGRATION FOR NEW GRADUATES, AND CONTINUE TO OFFER RESOURCES AND SUPPORT BASED ON EVALUATIONS AND NEEDS EXPRESSED BY THE STUDENTS AND GRADUATES THEMSELVES. COMMUNICATIONS: MEDICC'S BIGGEST POLICY WIN WAS THE RESULT OF TWO SOLID YEARS OF POLICY AND COMMUNICATIONS WORK.
