To enhance the quality of life of undeserved persons by utilizing research, education and program activities which will improve lifestyle, behavior and responsible health choices. Focus areas include independent living services for older adults, high risk and disease state management services and intergration of rehabilitation and specialty services with health and fitness activities.

ReasonableCauseExplanation/ExplanationTxt0All of our programs and services were terminated on September 30, 2015. We were unable to completely close all of the accounting records until 2016. An extension for the 2015 990 return was filed on May 12, 2016, which extended to August 15, 2016, but was thought to extend to 11/15/16. An additional request to extend was not accepted. The reason for the failure to file our 990 form in a timely manner was completely due to the failure of the organization that was providing our accounting services. They explained that they were unable to provide our financial information because of continued turnover of the staff that was assigned to complete our books. The accountant assigned to our account, said their situation was further complicated when the controller of the organization was fired and not replaced for months. The remaining staff was hard pressed to cover all their other work. He said our account, which was small compared to their others, was placed at the bottom of their priority list. Apparently, my phone calls did not add enough pressure to produce faster results. I finally received the completed 2015 financial information on June 1, 2017.
