The lifespan audit committee always reviews the independent audit report prior to approval by the board. The committee also reviews the form 990 prior to submission to the board and the irs.

Form 990, Part VI, Section B, Line 12C

Board members complete a conflict of interest form at the beginning of each board year. If a board member has a potential conflict of interest on a board issue, they reveal that conflict and abstain from discussion and any subsequent vote.

Form 990, Part VI, Section B, Line 15

Compensation decisions for the ceo and key employees involves completion of an employee performance evaluation that is discussed with the employee. The board executive committee is responsible for the ceo evaluation. All board members are surveyed, results are compiled and shared with ceo. Salaries for comparable positions are reviewed prior to determining the salary. The board accepts the evaluation and approves the salary increase. Evaluation and salary decisions of key employees other than the ceo is an administrative function. Each employee receives an annual evaluation. Salaries are commensurate with area salary surveys.

Pathways to health: a new program being offered by lifespan that help individuals who struggle with health challenges to learn and practice good self-management skills to live a better life. Pathways to health workshops focus on diabetes and chronic health issues and other workshops are being developed in connection with partners and needs in the community.

PART IV, LINE 2B:

Funds that are being held in a custodial nature related to the consumer credit counseling and payee programs are reported as restricted cash and the corresponding liability is reported as custodial deposits.

PART V, LINE 4:

Investment philosophy - the endowed funds established are given constant stewardship by the board of directors and professional staff to ensure that their intended purposes are carried out within a constantly changing environment. The lifespan endowment fund is administered by the foundation. Investments are made in a manner consistent with the foundation's investment policy to produce income and capital gains through a diversified portfolio. Spending policy - the purpose of a spending policy is to offer donors and beneficiaries a projected amount of money that will be available for distribution on an annual basis, while preserving the fund itself for perpetuity and allowing for fund growth. A policy allows donors to plan for and budget their annual charitable gifts. For organizations, a spending rate policy offers the chance to plan for and budget income from investments. The lifespan endowment fund is administered by the foundation. The organization requests that disbursements shall be made to support the operative and associated needs of the organization, 5% of the fund balance, computed on a 12-quarter rolling average, to be paid annually to the organization.

DURING THE YEAR 75 NEW INDIVIDUALS WERE ADDED TO THE PROGRAM.
